Off-switch 상태의 인공와우 이식 아동에 대한 청각 재활프로그램 적용 후 청각통합능력 변화에 관한 연구

A Study for the Variation of Auditory Integration Ability for Cochlear Implanted Children during Off-switch State

본 연구는 인공와우 이식 아동의 수술 후 off-switch 기간동안에 재활 프로그램을 개발하여 적용하여 봄으로써 청각통합능력에 미치는 영향을 알아 본 것이다. 연구의 대상은 예비 실험 아동 2명과 본 실험 아동 4명으로 하였다. 예비 실험 아동 2명은 후천성 청각장애 아동으로써 본 실험의 치료 프로그램을 개발하는데 기초를 마련하기 위해서 실험한 집단이다. 실험 설계는 사전·사후 설계로 하였다. 치료 프로그램은 시각적 자극단계, 촉각적 자극 단계, 동작 모방 단계로 나누었다. 청각통합능력에 대한 평가는 아동의 부모님들이 청각통합능력 검사(이상흔 외, 2003)로 실시하였다. 실험집단에서 청각통합능력에대한 변화가 있는가를 살펴보기 위해서 청각통합능력을 검사한 결과, 소리의 산출 양이 변화하는 영역, 조용한 장소에서 아동이 자발적으로 반응하는 영역, 집단의 환경음에 자발적으로 반응하는 영역에 변화가 나타났음을 알 수 있었다. 청각통합능력에 대한 평가 결과, 실험집단에 있는 아동 모두 치료 전보다 치료 후에 청각적 통합능력이 향상되는 긍정적인 변화가 나타났다. 따라서 off-switch 상태의 청각 재활프로그램은 청각통합능력을 개선시켜서 발성을 촉진하는 효가를 가지고 있다.

The purpose of this study was to investigate the effects auditory rehabilitation program for cochlear implanted children during off-switch state. The effects was measured with auditory integration ability. Ten subjects participated in the study. The subjects were free of intellectual, emotional, behavioral or visual problems. Two subjects participated in preliminary experiment and 4 in the main experiment. The off-switch program was composed of 3 activities : visual feedback, tactile feedback, and body movements. RION HU-05 AUDIO SPECTORA and VST 2002 were used as the visual feedback devices. RION AU 02B Vibratory Sensation Meter was used as the tactile feedback device. Body movement activities were developed by the author based on the Verbotonal method (Asp, 2000). The intervention took place twice a week. Each Session lasted 40 minutes. A t-test was performed pre- and post-treatment on variation of auditory integration ability. The results was that auditory integration ability of experimental group was significantly improved after the treatment. In conclusion, the off-switch auditory rehabilitation program was effective in developing the auditory integration ability.
