양극성 장애 환자에서 Bupropion 투여 후 발생한 하지불안증후군 1예

A Case of Restless Leg Syndrome Induced by Bupropion in a Patient with Bipolar Disorder

양극성 장애 진단 하에 외래 치료 받던 35세 여자 환자가 bupropion SR 150mg을 추가 복용한 뒤 3일 후부터 저녁이면 양쪽 다리가 저리고 뭔가 스멀스멀 올라오는 느낌이 들고 다리를 자꾸 움직이지 않으면 안된다는 불편감을 호소하였다. 양 하지의 불편감은 특히 밤에 침대에 누웠을 때 가장 심해져 환자는 잠들기가 힘들었고 침대에서 일어나 왔다갔다하면 불편감이 조금 완화되었으며 낮에는 다소 호전되는 양상이 매일 지속되었다고 하였다. 저자들은 환자의 임상 양상을 관찰한 결과 하지불안증후군으로 진단하였고 발생 10일 후부터 clonazepam 0.5mg을 투여한 후 하지불안 증상은 호전되었다. 여러 선택적 세로토닌 재흡수 역제제(SSRI)에 의한 하지불안증후군이 보고되고 있고 이는 serotonin계의 자극에 의한 dopamine hypoactivity 가설로 설명되고 있지만 bupropion SR을 투여한 후 발생한 본 증례는 이 가설로 설명되지 않아 하지불안증후군의 병태 생리는 더 복잡한 기전에 의한 것임을 시사한다. 본 증례는 bupropion으로 인한 하지불안증후군의 첫 번째 보고라는 점에서 의미가 있고, 앞으로 항우울제 투여 후 발생하는 하지불안증후군에 대한 면밀한 관찰과 연구가 필요할 것으로 생각한다.

A 35 year-old female patient with bipolar disorder, single manic episode in remission, were being followed up in the outpatient clinic regularly. Since she complained of depressive symtpoms, bupropion SR of 150 mg was adminstered. On 3 days after administration of bupropion SR, she complained of creeping and crawling sensation on both legs which was aggravated at night and attenuated with movement. She also had a difficulty in falling a sleep becuase of the discomfort of both legs and the urge to move. We observed these symptoms during 1 week, but her discomfort was not improved at all. Therefore we considered her symptoms as restless leg syndrome (RLS) induced by bupropion SR, and started clonazepam of 0.5 mg for the control of her discomfort on 10 days after bupropion SR administration. On the day after addition of clonazepam, her discomfort of both leg began to subside. Many cases of SSRl-induced RLS have been reported and these were explained by the dopamine hypoactivity related with stimulation of serotonin system. But, RLS induced by bupropion in this case cannot be explained by that hypothesis, so which suggested that more than one neurotransmitter system involves the pathogenesis of RLS. On our knowledge, this is the first one reporting RLS induced by bupropion.
