섬망치료에 관한 올란자핀 근주와 할로페리돌 근주의 효과와 안정성에 대한 비교 연구

Comparison of the Clinical Efficacy and Safety between Intramuscular Olanzapine and Intramuscular Haloperidol Injection in the Treatment of Delirium

&nbsp;&nbsp;본 연구는 섬망 환자에서 비정형 항정신병 약물인 올란자핀과 전형 항정신병 약물인 할로페리돌 근육주사의 효과와 안정성을 비교하기 위하여 시행 되었다. 62명의 섬망 환자를 올란자핀과 할로페리돌 근육 주사군으로 무작위 할당하였으며 연구자의 판단에 따라 7일간 약물의 용량을 조절하여 투여 하였다. 환자의 임상 양상의 변화는 DRS-R-98(Delirium Rating Scale-revised-98)과 CGI-S(Clinical Global Impression-severity)를 사용하여 매일 평가 하였다. 약물부작용은 SAS(Simpson-Angus Scale), BARS(Barnes Akathisia Rating Scale), AMIS(Abnormal Involuntary Movement Scale)로 평가 하였으며 환자가 주관적으로 호소하는 모든 증상을 기술 하였다.<BR>&nbsp;&nbsp;DRS-R-98은 시간이 경과함에 따라 유의하게 감소하였고(F=26.02, P＜0.0001) 그룹 간의 차이(F=0.048, P=0.829)나 교호작용은 없었다(F=5.64, P=0.725). 부작용의 발생 빈도에 있어서는 두 군 간의 유의한 차이는 없었다. 결론적으로 본 연구는 올란자핀 근육주사가 할로페리돌 근육주사보다 섬망 치료에 있어 더 우수하다는 결과를 얻지는 못하였다. 하지만 올란자핀과 할로페리돌 근육주사가 섬망 환자의 치료에 있어 둘 다 유용하고 안전한 치료 방법임을 시사하고 있다.

&nbsp;&nbsp;The present study aimed to provide preliminary data on the effectiveness and tolerability of intramuscular (IM) olanzapine and IM haloperidol for patients with delirium. Methods：Sixty-two patients with delirium were randomly assigned to either olanzapine IM or haloperidol IM groups, with a flexible dosing schedule for 7 days. The Delirium Rating Scale-revised-98 (DRS-R-98), clinical global impression-severity (CGI-S) were assessed daily. The Simpson-Angus Rating Scale (SAS), the Barnes Akathisia Rating Scale (BARS) and the Abnormal Involuntary Movement Scale (AIMS) were used for the assessments of side effects. Results：The DRS-R-98 scores were significantly decreased over time (F=26.02, P＜0.0001), without any significant group differences (F=0.048, P=0.829), and time by treatment group interaction (F=5.64, P=0.725). There were no serious adverse events in both groups. The scores on the SAS, BARS, and AIMS were not changed significantly during the study. Conclusions：This study showed that olanzapine IM did not show any superior efficay and safety compared with haloperidol IM in treatment delirium. However our study suggested that either olanzapine IM or haloperidol IM would be effective and tolerable. Adequately powered studies with a head-to-head comparison design will be mandatory to draw any definite conclusion.
