잠재적 공공 의료서비스에 대한 소비자 선호분석

Analyzing Consumer"s Preference to the Potential Public Health-Care Service

  당면한 민간 및 공공 의료서비스간의 불균형을 해결하고 향후 바람직한 공공 의료서비스 정책방향을 수립하기 위해서는 소비자 효용이론을 바탕으로 한 공공 의료서비스 프로그램의 사전적 설계가 우선적으로 이루어질 필요성이 있다. 이에 본 연구는 컨조인트 분석법을 적용하여 공공 의료서비스의 여러 양적ㆍ질적 속성들에 대한 소비자의 지불의사액을 도출하고자 한다. 이를 위해, 공공의료시설, 보험급여 혜택, 진료대기시간, 본인부담율의 4가지 속성을 고려하였다. 모든 추정치들은 통계적으로 유의했으며 개별 속성에 대한 한계지불의사액(MWTP, marginal willingness to pay) 계산 결과는 다음과 같다. 진체의료시설 중 공공의료시설의 비중 증가에 대한 MWTP는 20.3(원/%)이었다. 보험급여혜택을 외래ㆍ입원, 수술진료 수준에서 예방접종 혜택을 추가시키기 위한 MWTP는 1,085.8(원), 여기에 MRI와 초음파 검사를 추가시키기 위한 MWTP는 5,464.3(원), 종합건강검진을 추가시키기 위한 MWTP는 5,617.4(원)으로 분식되었다. 마지막으로 진료대기시간의 절감과 본인부담을 저감에 대한 MWTP는 각각 40.5(원/분) 및 73.9(원/%)으로 추정되었다. 이러한 연구결과는 공공 의료서비스의 개선정책과 관련한 의사결정과정에서 유용한 정보로 활용될 수 있다.

  For solving the unbalance between public health-care service and private health-care service, and establishing desirable policies of public health-care service, to design a public health-care service program based on consumer’s utility theory is needed. This study attempt to analyze willingness to pay (WTP) for public health-care service. Especially, this study applies a conjoint analysis to measuring marginal WTP for each attribute (ratio of public health-care service, insurance coverage, waiting time and one’s own burden for health-care service) of public health-care service. As a result, marginal WTP for increasing ratio of public health-care service by 1% is 23 Korean won, marginal WTP of insurance coverage for changing from diagnosis and hospital treatment to protective inoculation is 1,085.8 Korean won. In addition, marginal WTP for decreasing wait time by 1 minute is 40.5 48,965 Korean won and marginal WTP for decreasing one’s own burden for health-care service by 1% is 73.9 Korean won. These results contributes to providing policy-makers with useful methodological framework and quantitative information in the decision-making process related to public health-care service.
