재발성 세균성 뇌수막염을 동반한 특발성 접형골형 뇌류 1례

저자들은 외상이나 수술 병력이 없는 환아에서 1년내에 2회의 세균성 뇌수막염이 발생한 후, 자세히 병력 청취를 하여서 뇌척수액 비루와 연관된 해부학적 기형을 의심한 후 발견된 특발성 접형골형 뇌류 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Encephaloceles are congenital defects in the skull through which menings and brain tissues herniated. Basal encephaloceles are found in 1 in 40,000 live births. Transsphenoidal basal encephaloceles are very rare, accounting for only 5% of all basal encephaloceles. Currently, most encephaloceles are diagnosed antenatally and present at birth. Postnatally, infants may present with CSF rhinorrhea, recurrent meningitis, headache, hypothalamicpituitary dysfunction. Some, particularly trasnssphenoidal encephaloceles are often clinically occult and diagnosis of the disease may be postponed up to adulthood. Surgical indications and approaches for transsphenoidal encephaloceles have been remained controversial. We report a case of CSF rhinorrhea and recurrent meningitis caused by transsphenoidal encephalocele.
