2002년 상반기 부산, 경남지역에서 유행한 무균성 뇌막염에 대한 임상적 고찰

A Clinical Study of Aseptic Meningitis in Busan and Kyoungsangnamdo from April to July, 2002

목 적 : 2001년 4월 중순부터 유행하기 시작하여 7월 하순까지 부산, 경남지역에서 집중 발생한 무균성 뇌막염의 임상 양상을 알아보고자 본 연구를 시행하였다. 방 법 : 2002년 4월 중순부터 7월 하순까지 부산 백병원 및 동래백병원 소아과에 입원하여 뇌막염의 임상 증상을 보이면서 뇌척수액 검사상 염증의 소견이 확인된 환자 184명을 대상으로 하였다. 뇌척수액 세균 배양 검사에서 균이 자라는 경우는 제외되었다. 대부분의 환자에서 뇌척수액 바이러스 배양 검사를 시행하였다. 결 과: 1) 환자의 발생 시기는 2002년 4월 1일부터 2002년 7월 31일까지였으며, 가장 많은 환자 분포를 보인 달은 5월이었다. 2) 성별 분포는 남아 123례(66.8%), 여아 61례(33.2%)로 남녀비는 2:1이었다. 3) 연령별 분포는 2세에서 16세까지였으며 평균 연령은 8.7±7.1세였고, 5-9세군에서 96례(52.2%)로 호발하였다. 4) 임상증상은 발열 175례(95.1%), 두통 169례(91.8%), 구토 165례(89.7%), 복통 59례(32.1%)였으며, 복통, 설사 등의 위장관계 증상이 77례(41.7%)로 비교적 많았다. 1례에서 횡단성 척수염, 1레에서 안면 신경 마비 증상이 동반되었다. 5) 발열의 기간은 평균 2.6일이었다. 6) 말초 혈액 검사상 백혈구 수는 9,829/mm³이었으며 다핵구가 평균 78.3%로 나타났다. 7) 뇌척수액 검사상 백혈구 수는 평균 301/mm³이었으며, 평균 당은 58.3 mg/dL, 단백은 36.7 mg/dL이었다. 8) 뇌척수액 바이러스 배양 검사상 echovirus 6가 6례, echovirus 9이 3례, echovirus 13이 1례, echovirus 25가 2례, echovirus 30이 1레에서 검출되었다. 결 론 : 2002년도 4월 1일부터 7월 31까지 부산, 경남지역에서 무균성 뇌막염의 대유행이 있었으며 원인 바이러스는 echovirus 6, 9, 13, 25, 30으로 추정된다.

Purpose : There was an outbreak of aseptic meningitis in Busan and Kyoungsang-namdo area from April to July, 2002. In this study, we reported the clinical manifesta-tions, laboratory data and causative viruses. Methods : 184 children with aseptic meningitis who had been admitted to Department of Pediatrics, Inje Paik Hospital in Kaegum and Dongrae between April and July 2002 were assessed. Virus isolation and serotype identification were performed by cell culture and reverse transcription-polymerase chain reaction(RT-PCR) of cerebrospinal fluid. Results : The peak incidence was noted in May. Male-to-female ratio was 2:1. The age ranged from 2 years to 16 years. Mean age was 8.7±7.1 years. Clinical manifesta-tions were fever(95.1%), headache(91.8%) and vomiting(89.7%). Mean duration of fever was 2.6 days. The numbers of peripheral blood leukocytes were 1,400-20,000/mm³ and mean value was 9,829/mm³. On cerebrospinal fluid examinations, leukocyte count was in range of 10-2,000(mean 301)/mm³, protein level in range of 10-196(mean 36.7)mg/dL and glucose level from 17 to 155(mean 58.3)mg/dL. Viruses were isolated 12(8.4%) out of 154 patients and serotypes were 6 cases of echovirus 6, 3 cases of echovirus 9, 1 cases of echovirus 13, 2 cases of echovirus 25 and 1 case of echovirus 30. Conclusion : Aseptic meningitis was prevalent in Busan and Kyoungsangnamdo area from April to July, 2002. We thought that causative viruses were echovirus 6, 9, 12, 25, 30.
