A Study on the Situations and Trends of the Supply and Service System of Fee-Charging Home for the Elderly in Japan

The purpose of this study is to investigate a supply trend of fee-charging home for the elderly after the introduction of nursing care insurance and its service system. The major results of this study are as follows: First, fee-charging home for elderly people can be divided into three types: nursing type, housing type, health type. It has been revealed that fee-charging home offers a wider range of choices according to an illustration of the elderly facilities and well-fare facilities in Japan in terms of the residents' health condition(X axis) and their expenditure(Y axis). Second, more than 90 percent of the fee-charging homes surveyed were opened after the introduction of nursing care insurance in 2000, and their supply has been rising sharply since 2003. 95.4 percent of the facilities(82 units) request no lump-sum deposits or less than 1000 ￥ for residence, and 90 percent of the facilities(78 units) charge less than 200,000 ￥ for a month. Third, ⓐmeal service: Personnel shortages and financial difficulties prevent meal service from reflecting individual tastes, preferences, and eating habits. ⓑ welfare system: 25 kinds of indoor and outdoor recreational activities and mutual interactions were accomplished. ⓒcare service: There were little facilities for those who were attacked with senile dementia after moving into the facilities or who were approaching death. Fourth, considering the fact that there have been little precedented studies on fee-charging home for the elderly since the introduction of nursing care insurance, this study would be of some significance as a basic material for studies on the relations between fee-charging home and nursing care insurance or on the service system of care-requiring residents. Nevertheless, this study has a minor limitation in reaching a remarkable result due to lack of available survey results and many of the surveys' obscure responses.
