저자들은 생검만큼 정확한 정보를 제공하며서도 통상적인 건성안 검사법처럼 간단히 시행할 수 있는 impression cytology를 이용하여 결막 상피세포의 변화를 알아보기 위하여, 건성안 환자 43명, 43안을 대상으로 결막상태를 분석하고 동시에 이를 BUT, Schirmer test, rose bengal 염색법의 결과와 비교하여 이들 검사법의 유용성에 대해 알아 보고자 하였다. 대상 환자들의 BUT, Schirmer test, rose bengal score의 평균은 건성안의 진단기준에 부합하였으나 impression cytology상에서는 정상으로 삼는 stage 0에서부터, goblet cell의 완전한 소실 및 심한 squamous metaplasia가 관찰되는 stage 4까지 다양한 양상을 나타내었고, 이러한 안구표면상의 변화에 따른 BUT, Schirmer test 결과는 통계적으로 유의한 차이를 보이지 않았으나 rose bengal염색법상 stage 0와 stage 4사이에선 의미있는 차이가 관찰되었다(p＜0.05). 이상의 결과로, 건성안을 진단하는데 있어 임상증상 및 통상적인 건서안 검사법과 함께 rose bengal 염색과 impression cytology를 이용하여 결막의 세포학적 특징을 종합적으로 평가하는 것이 보다 정확하리라고 생각되었다.(한안지 35 : 362~370, 1994)
We evaluated the conjunctival status by impression cytology in 43 eyes of 43 patients with dry eye syndrome, which has been known to provide useful information as like as biopsy and simple and easy as convential clinical measures for dry eye syndrome. Also, the findings of impression cytology were compared with values of BUT, Schirmer test, rose bengal staning and finally we considered whether these diagnoistic tests are clinically useful or not As a result, the cytologic features are variable in view of squamous metaplasia(stage0-4) while values of BUT, Schirmer test, rose bengal were compatible with diagnostic criteria for dry eye syndrome. There were no differences in the values of BUT, Schirmer test among each stages of impression cytology, but was not in rose bengal scores between sgate 0 and 4, statistically(p＜0.05). This study suggests that the recognition of conjunctival cytologic features using impression cytology and rose bengal staining as well as present clinical symtoms and performing the conventional methods, may be important in diagnosis of dry eye syndrome(J Korean Ophthalmol Soc 35 : 362~370, 1994.)
