저자들은 중앙대학교 부속병원 안과에서 본태성 안검경련증 및 반측안면경련증이 있어 botulinum toxin 주사 치료를 받아온 환자들 중, 고혈압이 있어 calcuium channel blocker를 복용해 온 환자들에서 임상적으로 toxin의 약효가 더 오래 지속되는 것을 관찰하였기에, 이들을 병용투여시에 약효가 과연 증가되는지를 알아 보고자 본 연구를 시행하였다. botulinum toxin 주사 치료를 받아온 본태성 안검경련증 및 반측 안면경련증 환자들을 대상으로 botulinum toxin 과 calcuium channel blocker 인 diltiazem을 병용투여하여, 투여 전과 후의 경련소실 지속시간을 비교하였다. Diltiazem 투여하기 전, 즉 botulinum toxin만 주사한 경우 본태성 안검경련증 및 반측 안면경련 환자에서의 평균 경련소실 지속시간은 각각 123.94±42.74일, 167.32±52.90일 이었으나, diltiazem 병용투여 후 163.83±53.07일(p=0.0446), 154.21±43.36일(p=0.2156)로 그 지속시간이 안검경련증 환자들에선 통계학적으로 유의하게 증가하였다. 본 연구는, toxin 주사시 diltiazem을 병용투여하는 새로운 치료방법을 제시한다는데 그 의의가 있으며, 이러한 치료방법은 toxin에 대한 항체형성으로 인하여 약효지속시간이 감소하는 경우나, toxin의 국소적 합병증이 우려되는 환자들에서 효과적일 것이라는 결론을 얻었다(한안지 36:1643~1648, 1995).
We investigated whether symptom-free period(mean response time) could be prolonged when diltiazem, one of the calcuium-channel blocker, as a adjuvant was administered with botulinum toxinin blepharospasm and hemifacial spasm. The patients were 10 blepharospasms and 20 hemifacial spasms who han been treating with botulinum toxin injection in our hospital. When they received only botulinum toxin for treatment of blepharospasm and hemifacial spasm, their mean response times were 126.94±42.74 and 167.32±52.90 days, respectively. After start combination therapy of diltiazem-botulinum toxin, mean response times were prolonged to 163.83±53.07(p=0.0446) in patients with blepharospasm, on the other hand, 154.21±43.36(p=0.2156) days in patients hemifacial spasm. This study suggests that a combination treatment of diltiazem and toxin may be effective especially when the mean response time is decreased due to antibody production and the local complications are possibly expected in some patients. Present study is meaningful in that it provides a new treatment modality for blepharospasm and hemifacial spasm(J korea Ophthalmol Soc 36:1643~1648, 1995).
