각막이식후 이식된 내피세포의 밀도 변화 및 중심부 각막두께 등을 장기간 관찰하여 이식편의 생존율에 미치는 영향을 술 전 각각의 원인 질환 및 거부반응 등에 따라 분석함으로서 알아보고자 했다. 1985년 6월부터 1998년 2월말까지 전체 층 각막이식 수술을 받았던 47명(57안)을 대상으로 각막두께, 각막내피세포수 등을 측정하였다. 대상안들은 술 전 진단에 따라 크게 염증군, 비염증군 및 거부반응군으로 3분되었다. 전체 이식편의 중심부 각막내피세포의 수는 술 후 각각 3개월째에 2040cells/mm², 1년째에 1399cells/mm², 5년째에 1013.7cells/mm², 10년째에 786.1cells/mm²로써 세 군 모두 이식 후 첫 1년까지는 각막내피세포수에 커다란 감소가 있다가 그 후로는 서서히 안정화되었다. 염증군과 비염증군은 술 후 1년 이후부터 감소율이 둔화되는 양상을 보이다가 그 후로는 서서히 안정화되었다. 거부반응군에서는 다른 두 군에 비하여 각막내피세포밀도에 많은 감소가 있었다. 결론적으로 술 후 거부반응 및 생존율에 가장 많이 영향을 미치는 것이 선행질환임을 잘 인식하여 수술전 환자의 선택에 더욱 신중을 기해야 할 것으로 사료되며 각막이식 후 적어도 1년까지는 각막두께 및 내피세포에 대한 세삼한 경과관찰이 필요하다고 생각된다(한안지 40:2111~2117, 1999).
The purpose of this study was to find out an effect of preoperative causative disease and rejection episodes on the survival rate of donor grafts by analyzing the changes of transplanted corneal endothelial cell density and central corneal thickness after keratoplasty though the long-term observation. We measured 채구두미 thickness, corneal endothelial cells of forty seven patients(and their fifty seven eyes), all of whom were operated from June 1985 to February 1988. We divided those largely into three groups - inflammatory group, noninflammatory group, rejection group - depending on the preoperative corneal status. Without any exception there had been a drop of corneal endothelial cell density for the first one year and came stability at a slow pace after that : 2040cells/mm² in the third month, 1339cells/mm² in first years, 1013.7cells/mm² in fifth year and 786.1cells/mm² in tenth year. The inflammatory and noninflammatory group showed increasing stability later on. A further decrease in corneal endothelial cell density was observed in the rejection group compared to the other ones. It could be concluded that an ever cautious selection of patients who seem to obtain the desired result after the surgery is all important because causative diseases in most cases have a decidedly significant impact on postoperative rejection episodes and survival rates. Also, a more attentive observation on corneal thickness, endothelial cell density uis needed at least until a year after the corneal transplantation(J Korean Ophtalmol Soc 40:2111~2117, 1999).
