서울시 저소득층 재가중증장애인 보건복지서비스 이용현황과 욕구 조사

A Survey of Utilization Status and Need of Health Care and Social Services for People with Severe Disabilities in Seoul

지역사회에 거주하는 저소득층 재가장애인들은 국가와 사회가 제공하는 다양한 보건의료 및 복지서비스가 존재함에도 불구하고 낮은 소득수준과 서비스 접근성의 미비, 정보 취득의 어려움등으로 서비스를 이용하는데 많은 어려움을 겪고 있다. 이에 본 연구에서는 저소득층 재가중증장애인들의 보건의료 및 복지서비스 이용 현황 및 욕구를 조사하여 실제 서비스를 이용 하는데 따르는 문제점을 알아보고 이들에게 요구되는 서비스의 종류와 형태를 파악하여 지원방안을 모색해보고자 하였다. 연구는 일상생활활동에 타인의 도움을 요하는 서울시 거주 저소득층 성인 재가 중증장애인 81명을 대상으로 설문조사로서 수행되었다. 설문조사는 연구조사원이 장애인 가정을 방문하여 준비된 설문지로 직접 조사하였다. 설문항목은 대상자의 일반적 사항, 장애유형 및 정도, 건강상태, 보건의료서비스 및 복지서비스 이용 실태 등 5개 부문으로 구성되었다. 연구결과대상 장애인들은 모두 낮은 소득수준과 생활수준을 나타냈으며 간병과 보호가 대부분 가족에 의존하고 있어 도우미 파견과 같은 인적 지원과 경제적 지원이 요구되었다. 상당수가 건강상의 문제를 가지고 있어 높은 질병유병율과 치료 욕구를 보였으며 과반수이상이 일반적인 보건의료서비스와 재활의료서비스를 이용하고 있으나 그 만족도는 낮았다. 충분한 치료를 위해서는 의료비 및 이동수단 지원, 보건의료기관의 관심과 배려를 강화할 수 있는 방안이 요구되었다. 보장구 급여에 대한 인지도도 낮았으며 비용 부담으로 보장구 구입에 어려움이 있어 정보제공 및 급여확대와 같은 보장성 강화방안이 요구된다. 지역사회 복지서비스에 관한 정보 및 접근성 미비로 인식 수준이 낮았으며 이용도와 만족도도 낮았다. 따라서 중증장애인의 처지와 특성을 반영한 효율적인 보건의료와 복지서비스 제공 방안이 수립되어야 하며 서비스 확대와 효과적 지원이 있어야 할 것이다.

This study was to survey the utilization status and need of health care and social services for low-income people with severe disabilities in community. The subjects were 81 adult people with disabilities living in Seoul who have some limitation of activities of daily living. The contents of survey included economic and social status, type and severity of disability, state of health, and utilization rates, facilities, and needs of general medical and rehabilitation services and social services in community. As a result of the survey, most of subjects were in poor economic and social status and their main care-giver was the member of their family. They have low health status, so have high rate of morbidity and need of medical care. But they have difficulty in using the medical and rehabilitation services because of limitation in moving and a burden of medical cost and other reasons. Most of them have low utilization and satisfaction rate of social welfare services in community because of low accessibility of related information and services. So, it is necessary to supply people with severe disability with care-giving, and health care and social services related to characteristics and severity of disabilities based upon the personal and financial supports.
