이식편대숙주질환은 이식된 골수에 포함되어 있는 세포독성림프구가 숙주의 부조직적합항원을 인식함으로써 야기되는 치명적인 질환이다. 생쥐를 이용한 실험동물모형에서 CD4 양성 T세포아형 혹은 CD8 양성 T세포아형을 이식하여 줌으로써 이식편대숙주질환을 야기시킬 수 있다. 최근의 연구결과들은 이식편대숙주질환에서 각질세포가 apoptosis를 거친다는 점을 시사하였다. 그러나 이를 지지하는 형태학적 자료는 여전히 부족하다. 본 연구는 연속박절편을 통하여 각질세포손상의 apoptosis를 밝히고자 수행되었다. 급성이식편대숙주질환은 고도로 정제된 CD4 양성 혹은 CD8 양성 T세포아형으로 유발하였고 전아형이 포함된 T세포를 양성대조군으로 T세포를 제거시킨 골수를 음성대조군으로 사용하였다. 연속박절편을 통한 투과전자현미경학적 관찰을 수행하였다. 본 연구는 apoptosis를 거치는 각질세포의 보이는 연속적인 형태의 변화를 보여준다. 바닥층세포와 바닥위층세포 모두 초기 apoptosis 소견을 보였으며 각질세포 내 당김원섬유가 바닥막과 인접세포로 부터 분리되는 것은 apoptosis를 거치는 세포에서 일반적으로 관찰되었다. 세포질농축의 진행과정도 제시되었다. 전 실험군에서 apoptosis를 거치는 세포를 계측한 결과 21일군 까지는 CD4+ 아형군에서 더 많은 세포가 사망하였으나 28일 이후에는 CD8+ 아형군에서 세포사망이 많았다. Apoptosis를 거치는 세포들의 대부분은 각질세포였으나 일부 효과세포의 apoptosis를 관찰할수 있었는데 특히 CD8+ 세포 투여 28일 및 35일 군에서는 효과세포의 apoptosis가 다수 관찰되었다.
GVHD(Graft-versus-Host Disease) results from the cytotoxic T lymphocytes from the bone marrow recognizing the recipient's minor histocompatibility antigens. In experimental murine models, either CD4+ or CD8+ T-cell subsets can cause GVHD, depending upon the particular strain combination utilized. Recent studies suggest that the keratinocyte undergo apoptosis in GVHD. However, morphological data supporting this concept are still lacking. The present study was undertaken in order to document apoptosis in experimental acute GVHD via sequential analysis of ultrastructure. Acute GVHD was produced across minor histocompatibility loci using appropriately matched murine strains. Acute GVHD was mediated with the use of highly purified preparations of donor CD4+ and CD8+ T-cell subsets. Whole Tcells were used as a positive control and T cell depleted bone marrow as a negative control. Conventional transmission electron microscopy was used to define apoptosis structurally. Sequential ultrastructure revealed that the keratinocyte underwent apoptosis in CD4+, CD8+ and whole T cell groups. This study demonstrates the sequential ultrastructure of the keratinocyte undergoing apoptosis from the beginning to the end. Both of the basal and the suprabasal keratinocytes show the morphology of early apoptosis, and the detachment of the tonofibril from the basement membrane and the adjacent cell was the general findings in the apoptotic cell. Sequences of the cytoplasmic condensation was demonstrated. Through ultrastructural quantitation the apoptotic indices were depicted in all the experimental groups. Characteristically, numerous lymphocytes underwent apoptosis in CD8+ groups at day 28 and 35.
