구개열아동 말의 객관적 평가 및 치료방법

Objective Evaluation and Therapy of Speech in Children with Cleft Palate

구개열아동은 대부분 1-2세 사이의 조기에 수술하면 말·언어장애가 발견되지 않는다. 그러나 일부 구개열아동 중에는 조기에 수술을 받았음에도 말·언어장애가 발견되는 사례가 있다. 구개열아동의 말장애는 공명장애, 음성장애 및 조음장애 등이다. 공명장애는 과비음 또는 저비음으로, 음성장애는 쉰 목소리로 그리고 조음장애는 성문파열음, 설측음화 및 인두음화 등으로 나타난다. 본 연구는 비음측정기, 스펙트로그램 및 Visi- Pitch등을 사용 객관적인 방법으로 구개열아동의 말능력을 평가하고, 치료방법을 전통적인 방법과 객관적인 방법으로 나누어 소개하였다. 본 연구에 참여한 정상아동은 평가장비 별로 종합하여 총 115명이었으며 구개열아동은 33명이었다. 연구결과 구개열아동의 비음도는 정상 아동보다 2배 이상 높게 나타나 과비음 정도가 큰 것으로 나타났다. VOT 및 모음 포먼트값을 비교 검사 한 결과 구개열아동의 VOT는 정상아동의 VOT보다 길게 나타났다. 구개열아동의 음질 및 조음반응시간은 Visi- Pitch를 사용하여 발성지속시간 및 jitter와 diadochokinetic rate를 측정하였으며, 그 결과 구개열아동은 정상아동보다 발성지속시간이 두 배 짧았고 jitter도 1 % 이상 나타났으며 diadochokinetic rate도 지연되었다. 이러한 실험적인 분석자료를 토대로 구개열 언어치료방법을 간접적인 방법, 반간접적인 방법 및 직접적인 방법으로 소개하였고 비음측정기에 의한 바이오피드백 훈련방법과 임상에서 사용하고 있는 구개열 언어치료방법인 발음보조장치의 사용법을 소개하였다.

Children with cleft palate often show speech problems even though they received early surgical operation. The typical speech problems of children with cleft palate are the resonance disorder , voice disorder and articulation disorder . The speech evaluation of cleft palate has been tested by subjective methods. The aim of present study was to obtain acoustic data of cleft palate speaker s. One hundred and fifteen normal children and 33 children with cleft palate participated in this study. Nasometer , Spectrography and Visi-Pitch were used to measure the hypernasality, voice color and voice quality separately. The acoustic characteristics of the speech in children with cleft palates were found as follows: (1) More than 50 % of the subject s with cleft palate showed the nasalance of the high vowel / i/ ; (2) the VOT of cleft - palate subject s was two times longer than that of normal children; (3) the maximum phonation time of children with cleft palate was about half of that of normal children; (4) the jitter of children with cleft palate was over 1.3 %; (5) the diadochokinetic rate of children with cleft palate was one- half of that of normal children.
