A Qualitative Study of an Assessment Model for Neurogenic Speech-Language Disorders

본 연구의 목적은 신경말·언어장애 평가방법에 대한 문헌 연구 및 이에 관련된 한국의 언어치료전문가들과 관련 분야의 전문가들을 대상으로 현재의 평가방법과 체계에 대한 심층 면담을 통해 기존의 평가 상황에 대한 문제점과 한국 현장에 적합한 신경말·언어장애의 진단모델을 제안하는 것이다. 방법: 이를 위해 이 분야에서 일하고 있는 다양한 기관과 소속의 언어치료전문가, 의사, 언어병리학 및 관련 학문분야의 교수 10명을 선발하여 질적 연구를 실시하였다. 면담은 신경말·언어장애 평가에 대해 개발된 질문지를 통해 이루어졌으며 그 자료를 전사한 후 질적 분석을 하였다. 결과 및 결론: 분석 결과 다음과 같은 3개의 주제를 도출하였다. 즉, 평가에 관련된 (1)다학문적 요인으로, 용어의 통일과 분야 간 원활한 의사소통과 협력진단체계의 구축; (2)평가환경 요인으로, 단계적 평가체계를 뒷받침할 수 있는 다양한 도구의 개발과 비공식검사의 활성화; 그리고 (3)평가자 요인으로, 임상가들의 질적 향상과 관심영역의 확대 및 활발한 임상연구이다. 이에 대한 전문가들의 구체적인 인터뷰내용을 제시하였으며, 제안된 평가 모델에 대해 추가적으로 논의하였다.

The purpose of this study was to conduct literature review for assessment and develop a Korean assessment model for neurogenic speech-language disorders. Methods: Interviews were performed with 10 specialists from different clinical settings and related areas. The results were transcribed and analyzed using a qualitative constant-comparative design. Results & Conclusion: Three main themes emerged from the analysis: (1) a multidisciplinary approach, (2) consideration of the assessment condition, and (3) clinician competency. Subthemes under the main three themes included problems in use of terminology, a team approach for assessment, systematic assessment, development of assessment tools, importance of informal tests, and an increased of the quality of speech-language pathologists. A model based on this study is proposed for appropriate assessments for the differential diagnosis of neurogenic speech-language disorders in Korea.
