치매의 유형 및 중증도에 따른 결속표지 특성

The Characteristics of Cohesive Device according to the Types and Degrees of Dementia

본 연구는 알츠하이머형 치매환자와 뇌혈관성 치매 환자의 결속표지 사용 특성과 치매의 중증도에 따른 결속표지 사용의 특성에서 어떠한 차이점을 나타내는가를 알아보고자 한다. 연구 대상은 65~79세의 치매 노인을 대상으로 알츠하이머형 치매(AD) 44명(CDR 등급별 10-14명), 뇌혈관성 치매(VaD) 40명(CDR 등급별 10명)을 대상으로 하였다. 연구 방법은 대화 형식의 발화를 유도하여 다양한 형태의 담화를 포함한 자발화에서 결속 표지를 분석 하였다. 연구 결과 치매유형에 따른 유의한 차이는 나타나지 않았으며 치매의 중증도에 따른 유의한 차이만 나타났다. AD 집단과 VaD 집단의 중증도에 따른 결속표지는 치매의심군에서 경도군으로 갈수록 평균 점수가 급격히 높아지다가 경도 이후 치매의 중증도가 심해짐에 따라 결속표지의 평균 점수가 점차 감소하는 경향을 나타내었다. AD 집단이 VaD 집단보다 전체적으로 결속표지의 사용이 높게 나타났으나 심도에서는 VaD 집단이 AD 집단보다 높게 나타났다. AD 집단의 경우 결속표지의 사용 빈도가 가장 높은 경도 치매군을 제외한 나머지 치매군 간의 유의한 차이가 나타났으며 치매의심군과 심도치매군, 중도치매군와 심도치매군에서는 유의한 차이는 나타나지 않았다. VaD 집단의 경우 치매 의심군과 나머지 치매군 간의 유의한 차이가 나타났다. 즉 경도, 중등도, 심도 치매군 간의 유의한 차이가 나타나지 않았다.

The characteristics of typical utterance of dementia are use of irrelevant words about information, repetition of platitude, and lack of ability to maintain subjects. It is hard to find these speech characteristics of dementia through standard speech tests. Therefore, the purpose of this study is to investigate the characteristics of cohesive device in the spontaneous speeches of dementia. The speeches were collected after a series of activities in order to control both quality and quantity of speech. Narrative discourse, procedural discourse, expository discourse and conversational discourse are included with promotive questions. Cohesive device was analyzed as an index of ability of surmising the cohesion of a subject. The results were as follows; Firstly, Group AD showed higher cohesive device than group VaD. But there was no significant difference. Secondly, in VaD group, cohesive device showed lowest score in the potential dementia group while the other three dementia groups didn't show any significant difference. Finally, in AD group, cohesive device showed highest score in the mild group which the other three dementia groups didn't show any significant difference.
