하지 절단 장애인의 요인별 특성과 균형자신감과의 관계 분석

Analysis of the Relationship between Factor-Characteristics and Balance Confidence of Lower Extremity Amputees

본 연구의 목적은 장애인과 재활에 있어서 기존 연구가 부족한 절단장애인을 대상으로 절단장애인의 요인별 특성 및 낙상과 및 활동 특이적 균형자신감에 대한 관련성을 분석하고자 실시하였다. 서울 및 서울근교에 거주하는 252명의 하지 절단장애인을 대상으로 설문지를 이용하여 절단부위 및 원인, 사회-인구통계학적 특성, 보행 및 낙상관련 특성을 조사하였으며, 기존의 균형관련척도보다 적용범위가 더 넓고 기능적인 면이 포함된 균형 측정도구인 활동 특이적 균형자신감(activities-specific balance confidence, ABC) 척도로 평가하였다. 본 연구에서 다음과 같은 결과를 얻었다. 첫째, 절단 부위에 따른 ABC 척도 점수에서 하퇴군의 평균 점수가 대퇴군보다 유의하게 높았다. 둘째, 절단 원인에 따른 ABC 척도 점수에서 비혈관성군의 평균 점수가 혈관성군보다 유의하게 높았다. 셋째, 하퇴 절단 중에서 비혈관성 절단이 혈관성절단에 비하여 ABC 척도 점수가 유의하게 높았으며, 비혈관성 절단 중에서 하퇴 절단이 대퇴 절단에 비하여 ABC 척도 점수가 유의하게 높았다. 넷째, 낙상 경험 및 낙상 두려움에 따른 ABC 척도 점수에서 낙상을 경험하지 않은 군과 낙상을 두려워하지 않는 군의 점수가 유의하게 높았다. 본 연구 결과를 통해 하지 절단장애인의 재활과정동안, 활동 특이적 균형자신감에 영향을 미치는 특성을 파악하고 이에 따라 적절한 기능적 훈련이 고려되어야 할 것으로 판단된다.

The purpose of this study was to investigate the socio-demographic factors, gait related factors, falling related factors, and the balance confidence of lower extremities amputees and to analyze the relationship between these factors and the balance confidence. For this purpose, the data were collected from 252 lower extremities amputees who reside in Seoul and in the suburbs of Seoul by utilizing self-administered questionnaire. To measure the balance confidence we used activities-specific balance confidence (ABC) scale. The following results were as follows: (1) The transtibial amputee group is significantly higher in ABC scale score than the transfemoral amputee group, (2) The amputee group due to nonvascular causes is significantly higher in ABC scale score than the amputee group due to vascular causes, (3) The nonvascular amputee group is significantly higher in ABC scale score than vascular amputee group. And the transtibial amputee group is significantly higher in ABC scale score than the transfemoral amputee group among the nonvascular amputee group, (4) The people who have not fallen group, the people who have not fear of falling group are significantly higher in ABC scale score. Through the results of this study, to understand the balance confidence in rehabilitation of lower extremity amputee is important. And balance training based on ABC is might be helpful for lower extremity amputee to during rehabilitation process and return to daily living.
