닫힌사슬과 열린사슬에서의 하지저항운동이 뇌졸중환자의 하지 근 활성도, 균형, 보행에 미치는 영향

Effect of a closed chain and open chain resistance exercise on lower extremity muscle activity, balance, gait for the patients with stroke

Objective: To determine the effects of lower extremity resistance exercise - close kinetic and open kinetic, on stroke patients' lower extremity muscle activity, balance and walking. Method: 36 patients suffering from hemiplegia as a result of stroke were divided into 3 groups of 12 persons each and put under Close Kinetic Exercise(CKE), Open Kinetic Exercise(OKE), and Manual Exercise(ME) for 30minutes a day, 5 times a week, for 6 weeks. Measurement tools used to determine lower extremity muscle activity were electromyogram, Korea version Berg balance scale(K-BBS), Timed up and go(TUG), 10meter walk velocity(10m WV) etc. Paired t-test was used to compare results before and after for each group, and one-way ANOVA was used to analyze and compare the difference between groups. Results: In comparing results for each group, all 3 groups showed a significant change(p<.05) before and after for muscle activity in K-BBS, TUG, 10m WV. In comparing between groups, close kinetic exercise displayed a significant enhancement(p<.05) in K-BBS, TUG, 10m WV for medial hamstring, lateral hamstring and vastus medialis. Conclusion: It can be concluded that CKE is more effective that OKE in improving a stroke patient's lower extremity muscle activity, balance and walking. Together with ME, this can be an effective treatment method that reduces personnel and time.
