요양병원과 노인성 질환

Long Term Care Center and Geriatric Illness

2008년 7월부터 노인 장기 요양 보험제도가 시행되었으며, 2009년 5월 21일 노인 장기 요양 보험법개정, 법률 제9693호가 제정되었고, 2011년 12월 8일 시행, 노인장기요양보험법[법률 제10785호, 2011-6-7 타법 개정]이 또 다시 개정 작업이 이루어져 오늘에 이른다. 그리고 2012년 2월 5일 시행, 법률 제11013호, 치매 관리법이 규정되었다. 이러한 법률적 배경과 더불어 현재의 요양 병원에 관한 질환 및 치료 대책에 관하여 검토하였다. 요양 병원에 입원하는 대상 환자는 법률적으로 한정되어 있다. 보건복지부 시행 규칙에 의거하여 만성 질환, 노인성 질환 및 수술 후 회복을 필요로 하는 상태에 국한된다. 실제 요양 병원에 입원하는 환자들의 대부분은 노인 환자들이며, 노인성 질환으로 인해 입원을 필요로 하는 상태이고, 만성병 환자나 혹은 외과 수술 후 회복을 위해 입원하는 경우도 노인환자에서 발생한 경우들이며, 이들의 대부분은 치매, 뇌졸중 및 파킨슨병 등 신경과 질환 환자들이다. 결과적으로 요양병원에서는 노인 환자들을 위한 병원이며, 만성적인 신경과 질환을 대상으로 하는 병원인 셈이다. 그리고 장기 요양 보험법에 의거한 등급 판정과는 상관성이 없다고 하지만, 요양원과 연계됨을 부정할 수는 없다. 그렇다면 노인 문제와 더불어 노인성 질환, 특히 신경계 질환을 다루는 병원으로서 노인 신경과학의 관점을 도입해야만 한다.

In 2008, it was incompletely set up the long term care system in Korea. Whereafter the act for long term care insurance was officially announced in Nov. 2011 and the act for dementia, Dementia Management Act, was drafted in Feb. 2012. In Korea health care system and public welfare system are slowly and step by step changed and improved. It make efforts to promote the welfare of the people, but is not provide good benefits for geriatric patients. In long term care center, there are so many elderly patients suffering from the neurological diseases such as strokes, dementias and Parkinsonism. Moreover, the neurological diseases have combined with multiple chronic diseases or geriatric problems (ex, gait and balance disorders, decreased nutritional status, constipation, depression, sleep disturbances, urinary incontinence, or joint pain). Principally, specialists including with neurologists or geriatric physicians are involved in these cases. In practice we are concerned about the special care plans of neurology and geriatric medicine in long term care center.
