노인병원 입원환자 보호자의 만족도에 미치는 요인

Multifactor Analysis of Carer Satisfaction in Geriatric Hospitals

연구배경: 최근 노인병원이 증가하고 있지만 노인병원 입원환자 및 보호자에 대한 연구는 부족한 상태이다. 이에 본 연구에서는 노인병원 입원환자 보호자의 만족도에 미치는 인자를 분석하여 향후 장기 입원이 필요한 환자의 전원시 요양기관 선택 및 노인병원 운영에 도움을 줄 수 있는 자료를 제공하고자 하였다. 방법: 연구대상은 노인병원에 1달 이상 입원한 환자의 보호자 131명을 대상으로 하였다. 연구의 설문지는 조사 대상자의 일반적인 특성, 일상생활동작 수준과 보호자의 병원선택요인, 입원 중 진료서비스 만족도 문항으로 구성하였으며, 병원선택요인과 만족도 문항은 리커트 5점 척도로 측정하였다. 설문조사는 각 대상자에게 설문지를 배포하고 직접 기입하게 하였다. 병원 만족도에 미치는 요인을 알아보기 위하여 상관분석, 순차적 다중회귀분석, 일원배치 분산분석을 시행하였다. 결과: 노인병원의 선택요인은 간병, 간호, 기능회복시설 항목 순으로 중요하게 생각하는 것으로 조사되었다. 노인병원의 만족도는 간호, 병원환경의 청결도, 냉난방 시설 항목 순으로 만족도가 높은 것으로 조사되었다. 전체적인 노인병원 만족도와 유의한 상관관계가 있으며 영향력이 큰 병원선택요인은 간병, 기능회복시설 순이었으며, 만족도 문항은 진료나 치료일정의 계획성, 편의시설의 편리성 순이었다(P＜0.05). 또한 향후 환자의 호전 기대도와 노인병원 만족도는 유의한 상관관계를 보였다(P＜0.05). 유병기간, 환자의 장애 정도 등은 전체적인 노인병원 만족도와 유의한 상관관계가 없는 것으로 분석되었다. 결론: 본 연구를 통하여 노인병원 환자 보호자의 만족도에 미치는 영향은 향후 환자의 호전 기대도가 중요하며, 간병, 기능회복시설에 대한 중요도를 높게 평가하는 경우에 만족도가 높음을 알 수 있었다. 또한 노인병원 환자 보호자의 만족도를 높이기 위해서는 진료나 치료일정의 계획성, 편의시설의 편리성을 높이는 것이 중요함을 알 수 있었다.

Background: There were not enough researches about patients and carers in geriatric hospital, although a lot of geriatric hospitals were established lately. The purposes of this study were to investigate the factors to select a geriatric hospital as long term care unit and to investigate the carer satisfaction of geriatric hospitals. Methods: Total 131 carers of patient who had been in a geriatric hospital for more than 1 month were surveyed. Our questionnaires were composed of general characteristics of patients, the level of activities of daily living, the factors on choosing a geriatric hospital and carer satisfaction of a geriatric hospital. We analyzed the characteristics of factors that influenced carer satisfaction of a geriatric hospital. Results: Total carer satisfaction was significantly correlated with a person tending the sick, functional improvement equipments in the factors to select a geriatric hospital and a favorable process of medical program, convenient facilities in the factors of carer satisfaction (P＜0.05). Total carer satisfaction was also significantly correlated with the expectation to be restore to health (P＜0.05). Conclusion: The carer satisfaction of a geriatric hospital were related with the expectation to be restore to health, and the consideration of importance about a person tending the sick, functional improvement equipments.
