제 2형 당뇨 환자에서 자율신경기능과 우울증과의 연관성

The Association between Autonomic Function and Depression in Type 2 Diabetes

연구배경: 제 2형 당뇨병은 합병증 예방을 위해 주의 깊은 관리가 요구되는 만성질환이다. 당뇨병성말초 신경 병증은 제 2형 당뇨 환자에서 우울증과 독립적인 연관성을 보이며 우울증은 당뇨 조절과 연관된 환자의 의학적 순응도를 저하시켜 부정적 영향을 끼친다. 본 연구의 목적은 말초신경 병증이 동반되지 않은 제 2형 당뇨환자에서 합병증 조기 진단을 위해 자율신경 기능과 우울증과의 관계를 알아보고자 하였다. 방법: 31명의 말초 신경 병증이 관찰되지 않은 제 2형 당뇨 환자를 대상으로 교감신경 피부 검사, Short Form 36 health survey (SF-36)와 Beck depression inventory (BDI) 검사를 시행 하였다. 결과: 교감 신경 피부 반응 검사의 잠시와 진폭은 각각 1.53±0.27 ms와 71.51±404.226 μV 였고, Short Form 36 health survey (SF-36) 검사의 신체 건강 지수와 정신 건강 지수는 각각 71.41±16.01와 70.01±19.42이었으며 BDI 검사 결과는 11.06±8.21이었다. 결론: 이변량 상관 분석 결과 당뇨병성 말초신경 병증이 동반되지 않은 제 2형 당뇨 환자에서 자율신경 기능과 우울증 사이에는 의미 있는 상관관계가 관찰되지 않았다.

Background: Type 2 diabetes is a chronic illness that requires management to prevent long-term complications. Diabetic neuropathy is a common complication that independently associated with depression in people with type 2 diabetes and the depression make type 2 diabetic people get poor medical adherence. The aim of this study was to assess the relationship between autonomic function and depression in type 2 diabetes without diabetic neuropathy for early detecting complication. Methods: Thirty one subjects without diabetic neuropathy are examined sympathetic skin response, Short Form 36 health survey (SF-36) and Beck depression inventory (BDI). Results: The latency and amplitude of sympathetic skin response was 1.53±0.27 ms and 471.51±404.226 μV. The score of SF-36 PCS, SF-36 MCS and BDI was 71.41±16.01, 70.01±19.42, and 11.06±8.21. Conclusion: In bivariate correlation analysis, there is no significant correlation between autonomic function and depression in type 2 diabetes without diabetic neuropathy.
