일측성 감각변화만으로 발현된 급성 횡단척수염

Acute Transverse Myelitis Presenting as Unilateral Pure Sensory Change

일반적으로 급성 횡단척수염은 하반신 불완전마비 또는 사지 불완전마비, 감각수준을 가진 감각이상, 그리고 괄약근 장애로써 나타나게 된다. 더욱이 최근 진단 기준에 의하면 급성 횡단척수염은 양측으로 나타나는 감각수준을 가진 신경학적 이상소견을 가지는 것을 기준으로 삼고 있다. 하지만 본 저자들은 유일한 신경학적 이상소견으로써 갑자기 발생한 좌측 상하지의 감각이상만을 가진 65세 여자환자를 경험하였다. 본 환자는 순수 감각 뇌졸중으로 생각되었으나 척수액검사와 뇌, 흉추 그리고 경추자기공명사진으로써 급성 횡단 척수염으로 진단되었다.

Generally, clinical symptoms of acute transverse myelitis (ATM) evolve as para- or tetraparesis, a sensory level or sphincter dysfunction. Furthermore, recent proposed criteria for ATM have generally included bilateral signs and/or symptoms in addition to defined sensory level. However we observed a 65-year-old woman presenting with sudden-onset left pure sensory change as a unique neurological deficit. The patient diagnosed as cerebrospinal fluid study and the magnetic resonance images of brain, thoracic and cervical spines.
