외부 간병인에서 나타나는 근골격계 통증

Musculoskeletal Disorders among Non-Standard Employed Caregivers in South Korea

연구배경: 외부 간병인력에서 나타나는 신체 부위별 근골격계 통증 양상 및 직무 특성과 대상자의 일반적인 특성에 따른 신체 부위별 근골격계 통증의 차이에 대해 알아보고자 하였다. 방법: 206명이 연구 조사 대상에 포함되었고, 일반적 특성에 대한 문항, 지난 3개월간 주로 간병했던 환자의 기능과 관련한 문항, 근골격계 통증의 위치와 강도, 특정 간병 활동 시 악화 여부 등에 대한 문항으로 구성된 구조화된 설문지를 이용하였다. 결과: 연구에 참여한 조사 대상자는 총 206명으로 전부 여성이었으며, 평균 연령은 56.3±5.3세였다. 전체 신체 부위 중 어느 한 곳 이상에서 통증을 호소하는 간병인은 전체의 85.9% (95% 신뢰구간 81.1∼90.7)였고, 신체 부위별로 어깨 55.8% (95% 신뢰구간 49.0∼62.7), 허리 49.5% (95% 신뢰구간 42.6∼56.4) 부위 통증을 가장 많이 호소하였으며, 그 다음으로 손 또는 손목 34.0% (95% 신뢰구간 27.5∼40.5), 무릎 21.4% (95% 신뢰구간 15.7∼27.0), 목 20.9% (95% 신뢰구간 15.3∼26.5) 부위 순이었다. 통계적으로 유의한 위험 인자로는 간병 경력, 통증으로 인해 일을 쉰 적이 있는지 유무, 환자 처치 업무 중 체위 변경, 침대에서 휠체어로 환자 이동 및 환자 씻기기였다. 결론: 외부 간병인에서 나타나는 근골격계 통증 유병률은 비교적 높은 편으로 확인되었다. 어깨, 허리, 손 또는 손목 등을 포함하여 여러 신체 부위에 걸쳐 통증을 호소하는 빈도수가 높았으며, 대상자의 특성 및 간병 업무에 따라서도 영향을 받을 수 있음을 확인할 수 있었다.

Background: To investigate the prevalence of musculoskeletal pain in several parts of the body and how these complaints vary with demographic characteristics, other risk factors in Korea’s caregivers. Methods: Self-reported questionnaires were given to each candidate and two hundreds six women caregivers were enrolled. Questionnaires included questions on location, severity of pain during the previous 3 months, aggravating factors, general characteristics of caregivers and patient's functional status. Results: All participants were female (100%) with an average age 56.3 years (SD 5.3). The prevalence (previous 3 months) of musculoskeletal pain was 85.9% (95% confidence interval [CI] 81.1∼90.7%). The most commonly affected region was the shoulder (55.8%), followed by the lower back (49.5%), wrist or hand (34.0%), knee (21.4%) and neck (20.9%). The statistical significant related factors were working experience, off work due to pain, changing a patient's position, transfering the patient from bed to wheelchair and bathing the patients. Conclusion: The prevalence of musculoskeletal pain in non-standard employed caregivers is high. Not only the shoulder and lower back but also several other regions of the body are frequently affected. The musculoskeletal pain was influenced by demographic factors and daily working tasks.
