팔이음뼈근육의 심한 위축과 위약을 동반한 당뇨병성 근위축증

Diabetic Amyotrophy with Severe Atrophy and Weakness of Shoulder Girdle Muscle - A Case Report -

10년 된 제 2형 당뇨병의 과거력을 가진 55세 남자가 10일 전 시작된 심한 통증을 동반한 우측 팔이음뼈 근육의 위약과 위축을 주소로 내원하였다. 경추와 어깨의 단순 방사선 촬영과, 전산화 단층 촬영에서는 이상 소견이 없었다. 환자의 진단으로 당뇨병성 근위축증이 의심되었으며, 이 질병에서 견갑상완부위를 침범하는 경우는 매우 드물었다. 대부분의 당뇨병성 근위축증 환자는 호전적인 결과를 보이나, 증례에서는 추적관찰 동안 임상양상의 호전을 보이지 않았다. 이에 저자들은 당뇨환자에서 드문 합병증인 당뇨병성 근위축증에서 팔이음뼈 근육을 일차적으로 침범한 증례를 경험하여 문헌 고찰과 함께 보고하는 바이다.

A 55-year-old man with a 10-year history of type 2 diabetes developed weakness and atrophy of right shoulder girdle muscle within 10 days. This was accompanied by severe right shoulder pain. Computed tomography of the cervical spine and X-ray films of the cervical spine and shoulder revealed no abnormality. The diagnosis of this patient was suspected to be a proximal diabetic neuropathy (diabetic amyotrophy). The involvement of the scapulohumeral region observed in this case is very unusual in proximal diabetic neuropathy. Most of diabetic amyotrophy in patients showed favorable outcome, but this patients showed no improvement. So we should have to consider the possibility of diabetic amyotrophy in patients remaining severe disabled.
