화장실에서의 노인 낙상 예방을 위한 안전손잡이 효과

Effectiveness of Bathroom Grab Bar to Prevent Falls in Elderly

본 연구는 그 중요성에 비하여 연구가 미흡하였던 노인 낙상의 예방 방안 중에서 재가 노인의 화장실에서의 낙상예방 방안 중 하나인 환경개선 방안을 개발하고 이의 효과 축정을 목적으로 하였다. 대조군이 없는 사전-사후 준실험 설계로 수행되었는데 수원시 권선구 보건소 방문간호팀의 관리일지 기록을 바탕으로 일일 평균 이용 노인 수 기준으로 상위 18개소 경로당을 선정하고, 이 경로당을 이용하는 60세 이상의 노인을 대상으로 사전조사 실시 후 6개월 간의 프로그램 중재 기간을 사후조사를 실시하였다. 사전조사의 분석 대상자 중 사후조사 시점까지 추적이 기능하였던 245명을 최종 연구대상으로 하였다. 조사 대상자 중 여자 81세 이상 연령층, 지난 1년간 낙상을 경험한 노인, 타인에 비해 자신이 잘 넘어지는 편이라고 인식하고 있는 집단에서 안전손잡이를 자주 사용하였다고 응답한 노인이 많았다. 안정손잡이를 설치한 이후 조사 대상자의 낙상 경험률은 변화가 없었으나, 안전손잡이를 사용한 후에 낙상에 대한 두려움 점수는 통계학적으로 유의하게 감소하였다. 이러한 연구의 결과를 토대로 화장실 안전손잡이는 낙상 예방에 간접적인 효과가 있다고 결론내릴 수 있다.

The purpose of this study was to understand effect of environmental intervention on preventing falls in bathrooms among the elderly. Bathroom grab bars were installed to prevent falling as the environmental modification intervention at 18 senior day centers in Kwonsungu, Suwon. Data collection was done through personal interviews of 245 day center users age of 60 or older using questionnaire before and after installing grab bars. Socioeconomic characteristics, experiences of falling in the previous year, subjective fall proneness, experiences of falling at senior day center, experiences of using this bathroom grab bar, and fear of falling were investigated to evaluate the bathroom grab, Among the subjects, people who are female, 81 years or older, fall victims in the previous year, and being of more prone to falling used this bathroom grab bar move often than others. There were no differences in the number of falling victims at these intervened senior day center bathrooms and after six months after installing grab bars. However, the fear of falling score decreased significantly after the intervention among previous fall victims who used the bathroom grab bar in the study subjects. Based on these results, the bathroom grab bar is indirectly effective on preventing fall in bathrooms among the elderly.
