Cavernous hemangioma is a benign congenital vascular malformation that can affect any part of the body. However, primary hemangioma of the thyroid gland is extremely rare with only six cases reported in the literature. We report here on a case of a cavernous hemangioma of the thyroid gland in a 66-year-old man. Magnetic resonance imaging (MRI) showed a huge exophytic mass measuring 17 cm, which had replaced the left lobe of the thyroid. The histologic examination of the surgical specimen revealed a huge cavernous hemangioma. (Korean J Endocrine Surg 2006;6:35-38)
