A Survey of Speech Sound Disorders in Clinical Settings

배경 및 목적: 국내에서는 말소리장애뿐 아니라 언어치료사들이 담당하고 있는 의사소통장애인 숫자에 대하여 정확하게 알려져 있지 않다. 교육과정 개발과 말소리장애인을 위한 정책수립을 위해 현황파악이 필요하다. 본 연구에서는 언어치료기관을 방문하는 전체 의사소통장애 인구에 대한 말소리장애의 비율을 조사하면서, 언어치료사와 기관 특성에 따른 차이를 조사하였다. 방법: 약 1천명의 언어치료 전공 졸업자에게 인터넷과 스마트폰을 이용하여 설문지를 배포하고, 회수된 457명의 응답내용을 분석하였다. 설문문항은 응답자의 교육배경, 임상경력 등에 관한 기본 질문 8문항, 의사소통장애 및 말소리장애 사례 수, 연령 및 성별 분포 등에 관한 질문 4문항 등이었다. 결과: 응답자가 지난 일주일 동안 평가하거나 치료한 의사소통장애 사례 수는 전체 7,093명이었다. 치료사 1인당 평균 15.5명을 일주일 동안 평가 또는 치료하는 것으로 조사되었다. 약 7,100명 중 44.1%는 말소리장애가 있는 것으로 분류되었으며, 11.6%는 다른 의사소통장애 없이 말소리에만 문제가 있는 것으로 나타났다. 말소리장애 아동은 4-5세가 가장 많았으며 거의 모든 연령대에서 남아가 여아의 두 배 이상을 차지하였다. 치료사의 근무기관이나 임상경력에 따른 차이는 없었으나, 교육배경에 따라서는 학부 출신 언어치료사의 담당 사례 수가 높은 것으로 나타났다. 논의 및 결론: 이상과 같은 말소리장애 인구에 대한 현황 파악은 교육과정 개발과 의사소통장애인을 위한 정책 수립을 위한 기초자료로 의미가 있다.

Objectives: It has been reported that children with speech sound disorders (SSD) compose a large percentage of caseloads for speech-language pathologists (SLPs). However, there is no available data on the prevalence of SSD in Korea. This study aims to investigate the prev-alence and current clinical status of SSD, especially focusing on SSD without other commu-nication problems (pure SSD). Methods: A survey was delivered to SLPs via email or smart-phone application; a total of 457 SLPs answered the survey. A descriptive analysis was used to examine caseload characteristics by gender and age of clinical cases. A comparative de-sign was used to examine SSD and pure SSD caseloads depending on a clinician's educa-tional degrees, years of experience, clinical setting, etc. Results: Forty-four point one per-cent of the speech-language service caseloads showed SSD, with 11.61% pure SSD. Boys made up 68% of those with SSD or pure SSD. The results revealed that the number of SSD caseloads was significantly different across clinicians' educational degrees, while not signif-icantly different across types of clinical settings and clinicians' years of experience. Conclu-sion: This survey provides a perspective on the current status of speech-language services and a foundation for future investigations on SSD.
