환자의 개인적 및 의료기관, 의료서비스제공자요인이 신뢰 및 역할 명확성, 치료순응에 미치는 영향

A Study on the Effects of Patients' Individual Factors, Medical Institute Factors, and Health Care Provider Factors on Trust, Role Clarity, and Compliance with Treatment

의료 서비스의 경우 서비스 품질과 고객만족의 향상 및 환자의 건강 목표 달성을 위해서 의료서비 스 제공자의 역량뿐만 아니라 환자의 순응이 매우 중요하다 . 그러나 최근 발전한 의학 기술에 비해 환자의 치료 순응도는 매우 낮은 수준이다 . 특히 의료서비스를 제공하는 과정에서 의사는 환자에게 여러 가지 처방을 지시하게 되는데 환자가 의사의 지시에 따르기 위해서는 우선적으로 자신의 역할 에 대하여 명확하게 인지하고 있어야 한다 . 따라서 의료서비스 제공자에 대한 환자의 순응도를 높이 기 위해서는 환자의 역할 명확성을 높이는 것이 반드시 필요하다고 할 수 있다 . 본 연구는 환자의 순응을 높이기 위하여 반드시 선행되어야 하는 요인인 환자의 역할 명확성에 초점을 두고 역할 명확성을 통해 환자의 치료 순응에 영향을 미치는 다양한 요인들에 대한 연구를 진 행하였다 . 연구 결과 , 환자의 개인적 요인 ( 질병의 심각성 , 건강관심도 ), 의료기관 요인 ( 의료기관 명성 , 시설수준 ), 의료서비스 제공자 요인 ( 의사전문성 , 의사의 환자에 대한 공감성) 은 환자의 순응중요 성 인식과 서비스에 대한 신뢰에 긍정적인 영향을 미치는 것으로 나타났다 . 또한 순응중요성 인식과 신뢰는 환자의 역할 명확성에 긍정적인 영향을 미쳤으며 , 환자의 역할 명확성은 환자의 치료순응에 긍정적인 영향을 미치는 것으로 나타났다 . 이러한 결과를 통해 환자의 치료 순응도를 높이고, 환자의 불순응으로 인한 사회 · 경제적 비용의 감소 및 의료기관의 환자관리능력 향상에 유용한 시사점을 제시할 수 있을 것으로 예상된다 .

In medical services, patient compliance is very important as well as the capability of health care service providers to improve service quality and customer satisfaction and to achieve patient health goals. However, compared to the level of recently developed medical technologies, that of patient compliance with treatment is very low. Doctors prescribe diverse medications and treatments to patients in the process of providing health care services. To follow the doctor's orders it is utmost important for a patient to compliance with health care providers it is necessary to increase patient role clarity. Focusing on patient role clarity, which is prerequisite for increasing compliance with treatment, the current study explored the effects of various factors that can affect patient compliance with treatment through role clarity. The results of the study showed that patients' individual factors (seriousness of disease, degree of health concern), medical institute factors (reputation, facility level), and health care provider factors (doctor's expertise, empathy) had positive effects on patients' awareness of the importance of compliance with treatment and their trust in health care service. In addition, patients' awareness of the importance of compliance and their trust in health care services significantly affected perceived role clarity, which was shown to have positive effects on patient compliance with treatment. The results suggest useful implications for raising patient compliance with treatment, reducing social and economic costs due to noncompliance, and improving the patient management capability of medical institutes.
