고주파 수술기구를 이용한 눈꺼풀테 종양 제거술의 장기 관찰

Long-Term Evaluation of Tumor Removal at the Eyelid Margin Using a Radio-Frequency Electrosurgical Unit

목적: 고주파 전기수술기를 이용한 눈꺼풀테 종양 제거술의 장기 추적관찰 결과를 알아보고자 하였다. 대상과 방법: 눈꺼풀테 종양으로 진단 받고 고주파 전기수술기구로 종양 절제술을 받은 후 5년 이상 경과된 환자 53명, 55안의 의무기록을 후향적으로 조사하였다. 나이, 성별, 생검 여부, 재발, 합병증에 대해 전화 문의 및 외래진료를 통하여 조사하였다. 결과: 남자는 22명 23안, 여자는 31명 32안이었다. 수술 당시 연령은 8.6세부터 76.8세로 다양하였다. 수술성공률은 55안 중 52예로 95%였으며 평균 8.9년의 경과관찰을 하였다. 55안 중 34안이 조직생검을 시행 받았으며 피내모반과 복합모반이 각각 13예로 가장 많았고 기저세포암, 지루각화증, 만성염증은 각각 2예로 5.9%를 차지하였다. 기타 다른 것으로는 편평유두종, 섬유증이 각각 1예(2.9%)씩 최종 진단되었다. 재발은 3예가 있었다. 눈썹 관련 합병증 이외의 심각한 합병증은 관찰되지 않았으며 첩모난생 및 첩모중생이 3예 발생하였다. 결론: 고주파 전기수술기구를 이용한 눈꺼풀테 종양제거술은 수술하는 데 있어 간편하며 높은 성공률을 보여주었다. 또한 장기간 경과관찰에서도 재발과 합병증이 적어 눈꺼풀테 종양 절제술에 있어 효과적인 방법으로 생각된다.

Purpose: To evaluate the long-term follow-up results of the eyelid margin tumor resection using a radio-frequency electro-surgical unit. Methods: We retrospectively reviewed the medical records of 53 patients (55 eyes) diagnosed with eyelid margin tumor and who underwent eyelid tumor resection using a radio-frequency electrosurgical unit more than 5 years prior. Age, gender, undergoing a biopsy, recurrence,- and complications were examined through phone questions and the out-patient department. Results: The study included 23 eyes of 22 males and, 32 eyes of 31 females, (age range 8.6-76.8 years). Surgical success rate was 95% (52 of 55 eyes) and mean follow-up period was 8.9 years. Thirty-four of 55 patients underwent a biopsy, and the most common cases were intradermal nevus and compound nevus (13 cases each), 2 cases of basal cell carcinoma (5.9%), 2 cases of actinic keratosis (5.9%), and, 2 cases of chronic inflammation (5.9%). Other cases included 1 case of squamous papilloma (2.9%) and 1 case of fibrosis (2.9%). There were 3 cases of recurrence. Eyelash-related complications after surgery occurred in 3 cases of trichiasis or distichiasis. Conclusions: Eyelid margin tumor resection using a radio-frequency electrosurgical unit is simple and showed high success rate. Additionally, it is considered as an effective method in the eyelid margin tumor resection due to low recurrence and compli-cation rates in the long-term follow-up.
