증식성 당뇨망막병증의 유리체절제술 후 신생혈관녹내장 발생의 예측 인자

Prognostic Factors for Neovascular Glaucoma after Vitrectomy in Eyes with Proliferative Diabetic Retinopathy

목적: 증식성 당뇨망막병증 환자에서 유리체절제술 후 신생혈관 녹내장 발생의 예측 인자를 알아보고자 하였다. 대상과 방법: 2008년 3월부터 2014년 5월까지 증식성 당뇨망막병증으로 양안 유리체절제술을 시행 받은 후 단안에만 신생혈관 녹내장이 발생한 14명 28안에 대해 신생혈관이 발생한 안과 발생하지 않은 안의 양안 비교를 통한 후향적 연구를 함으로써 당뇨망막병증의 유리체절제술 후 신생혈관 녹내장 발생에 영향을 줄 수 있는 여러 가지 개인적인 혼란 변수를 배제하여 증식성 당뇨망막병증의 유리체절제술 후 신생혈관 녹내장 발생의 예측 인자에 대하여 알아보았다. 대상 환자들의 신생혈관 녹내장 발생안과 발생하지 않은 안 간의 임상적 특징 및 술 전, 술 중, 술 후 소견에 대해 Wilcoxon matched-pairs signed-rank test와 Chi-square test를 이용하여 비교하였다. 결과: 술 전 시행한 망막 전위도 검사에서 명순응하 추체반응의 b파 진폭은 신생혈관이 발생한 안에서 유의하게 감소하였다(p＜0.05).결론: 술 전 시행한 전체 망막전위도 검사에서 명순응하 추체반응 b파의 진폭이 감소된 경우 유리체절제술 후 신생혈관 녹내장의 발생 가능성이 높으므로 면밀한 관찰이 필요하다.

Purpose: To investigate the prognostic factors for neovascular glaucoma after vitrectomy in eyes with proliferative diabetic retinopathy. Methods: In the present study we retrospectively reviewed intraindividual and interocular differences in 14 patients (28 eyes) who underwent pars plana vitrectomy for proliferative retinopathy with vitreous hemorrhage and having only 1 eye neovascular glaucoma. The patients underwent vitrectomy between March 2008 and July 2014 at Maryknoll Hospital. The patient clinical da-ta on preoperative, intraoperative and postoperative factors were compared. Statistical analysis was performed using the Wilcoxon matched-pairs signed-rank (Mann-Whitney) test and chi-square test to evaluate the significance of differences within the patient groups. Results: The decrease of photopic b-wave amplitudes on the preoperative electroretinogram significantly correlated with the de-velopment of neovascular glaucoma after vitrectomy for proliferative retinopathy with vitreous hemorrhage (p > 0.05). Conclusions: Based on the results from the present study, decreased photopic b-wave amplitudes on preoperative electroretino-gram is an effective prognostic factor for the development of neovascular glaucoma after vitrectomy for proliferative retinopathy with vitreous hemorrhage.
