뇌병변이 확인된 환자의 시야 이상 분석

Analysis of Visual Field Defect in Patient with Brain Lesion

목적: 안과를 내원하여 뇌병변이 진단된 환자의 시야검사 소견을 분석해 보고자 하였다. 대상과 방법: 2005년 1월 1일부터 2014년 6월 30일까지 안과에 내원하여 임상적으로 뇌병변이 의심되어 뇌자기공명영상을 촬영한환자 중 뇌병변이 확진된 환자를 대상으로 의무기록을 후향적으로 조사하였다. 결과: 55명(남:여=30:25) 중에서 시각경로와 연관된 뇌병변의 전형적인 시야 이상 소견인 대칭적 반맹 소견을 보인 경우는 31명이었다. 비전형적인 시야검사를 보인 경우는 24명으로 뇌병변이 있으나 정상시야를 보이는 경우 20명, 양안의 주변부 시야협착을 보이는 뇌경색 2명과 다발성 경화증 1명, 단안의 하측 수평시야결손을 보이는 비인후암 1명이 있었다. 결론: 뇌병변이 있는 환자에서 시야 검사상 비전형적인 소견이 동반되는 경우도 있었다. 따라서 시야 검사에서도 주변부 시야협착소견이나 단안에만 국한된 시야 이상소견이 보이는 경우 드물지만 뇌병변에 대한 가능성도 고려해야 할 것으로 생각된다.

Purpose: In this study we analyzed the results of visual field tests in patients diagnosed with a brain lesion in an ophthalmology clinic. Methods: The medical records of 55 patients who underwent visual field examinations and diagnosed with brain pathology using magnetic resonance imaging (MRI) from January 1, 2005 to June 30, 2014 were retrospectively investigated. Results: Of the 55 patients (M:F = 30:25), 31 displayed symmetrical hemianopsia, a typical visual field defect associated with brain lesions. Twenty-four patients showed atypical visual field defect (20 showed normal visual field despite the brain lesion, 3had bilateral narrowing of the peripheral visual field in 2 cerebral infarctions and 1 multiple sclerosis, and 1 had horizontal inferior visual field defect in 1 eye from nasopharyngeal cancer). Conclusions: Patients having a brain lesion with the optic tract invasion may present with atypical visual field defects. The peripheral visual field constriction and horizontal inferior visual field defect in 1 eye could be the visual field defect in patients with a brain lesion.
