일본 개호보험의 개혁과 예방급여로서 지역포괄지원센터의 역할

A Study on Innovation of Long-term Care Insurance in Japan and Operation of Community Total Care Support Center

본 논문은 일본의 2005년에 개정된 개호보험에 대하여 개괄적인 내용, 그 중에서 예방급여를 중심으로 하는 지역포괄지원센터에 대하여 고찰하고, 앞으로 우리나라 노인 장기 요양보험의 예방급여를 실시함에 있어서 시행착오를 줄이는 토대를 마련하는 것이 목적이다. 일본 개호보험의 개혁은 지역포괄지원센터에서 예방급여를 실시하는 것인데, 예방급여의 신설로 인하여 대상자들의 등급을 요지원 1ㆍ2를 유도하거나 머물러 있도록 하고, 이전의 다양한 사업의 효율성이라는 의미에서 통폐합하여 오히려 질을 떨어뜨리는 결과를 초래하였다. 또 공익성 차원에서 지방자치단체의 직영이 타당하지만 위탁이 많으며, 다양한 방법으로 이용자에게 부담을 전가시키고 있다. 우리나라 노인 장기 요양보험의 시사점은 지역포괄지원센터가 예방급여를 종합적, 체계적으로 제공하고 있다는 점, 예방사업은 효율성을 위하여 중심기관과 분리되어야 된다는 점, 서비스의 질을 높이기 위하여 적절한 전문 인력의 배치와 확보가 필요하다.

Japan is one of the longevity countries in the world. Above all, the long-term care of elderly people became a serious issue due to the rapid increase of the post-aged population over 75 years. This thesis examines the overview of long-term care insurance of Japan which was revised in 2005, especially the community total care support center centered on prevention benefit. The purpose is to prepare a foundation for reducing trial and error when enforcing the prevention service of long-term care insurance for the elderly in Korea afterwards. The innovation of the long-term care insurance in Japan is to enforce prevention service at community total care support centers. This new establishment of prevention service induced or maintained the ranks of subjects at lever one and two. The merge from the previous sense of efficiency of various businesses caused quality deterioration instead. Also, direct management of local self-government from a public interest level is logical but many cases are on consignment, and a burden is passed onto users through various ways. Implications of the long-term care insurance in Korea are that the community total care support center provides a general, systematic prevention service, prevention business should be separated from the central institution for efficiency, and appropriate arrangement and secure of professional manpower are required to improve service quality.
