국내 뇌성마비 및 지적장애 아동의 재활승마 효과성에 대한 메타분석

본 연구는 재활승마가 뇌성마비 및 지적장애 아동의 신체와 사회ㆍ정서에 미치는 효과에 관한 실험연구를 메타분석했다. 검색된 전체 177편의 연구에서 PRISMA 선별과정과 내적타당성 조건을 충족한 14편의 연구가 분석대상이 되었다. 선별된 연구는 총 173건의 효과를 보고했다. 분석은 PRISMA, CRF, forest plot, funnel plot, Egger’s regression, meta-ANOVA, Cochran’s Q, I2 등을수행했다. 분석결과는 첫째, 메타분석은 재활승마가 뇌성마비 및 지적장애 아동의 신체와 사회정서의 개선에 높은 효과가 있음을 보였다. 둘째, 승마기구 프로그램의 효과는 동질성을 보였으나,승마기구를 제외한 재활승마 프로그램은 효과에서 동질성을 확보하지 못했다. 셋째, 재활승마가 뇌성마비 아동에게 미치는 효과는 동질성을 유지했지만, 지적장애 아동에 미치는 효과는 이질성을 보였다. 넷째, 재활승마가 장애아동의 대소동작 변화에 미치는 효과는 동질성을 보였다. 하지 만 사회정서와 행동체력의 변화에 미치는 효과는 이질성을 보였다. 본 메타분석은 재활승마의 내적타당성과 외적타당성에 관해서 기존 연구를 계량적으로 검증했으며, 재활승마 연구의 향후방향에 대해서 논의했다.

The present meta analysis systematically reviewed empirical studies on effect of therapeutic horseback riding(THR) in children with cerebral palsy(CP) or intellectual disability(ID). The 14 out of 177 studies, fulfilled PRISMA and design conditions for internal validity, were selected for meta analysis. Those selected studies verified 173 outcomes, which were classified into gross-micro motion, social-emotional effect, and active physical fitness effect. We performed PRISMA, CRF, forest plot, funnel plot, Egger’s regression,meta-ANOVA, Cochran’s Q, and I2 analysis. Followed were the selected results. First, the study results included in meta analysis showed the effectiveness of THR on gross-micro motion, social-emotional aspect and active physical fitness in children with CP or ID. Second,although THR kept homogeneity of effects on CP children. it showed heterogenous effect on ID children, which suggested more studies for external validity. Third, effects of riding machine, included in THR as sub-program, fulfilled the condition for external validity. Fourth,effect of THR on gross-micro motion in children with CP or ID substantiated external validity. However effect on social-emotion aspect and active physical fitness showed low heterogeneity. The present study systematically reviewed internal and external validity of THR and, based on results, suggested a few encouraged study area of THR.
