The Collision accidents attributed to Tanker ships account for 24 of all marine collision casualties(2008-2012). Also, it is remarkable for the Tanker ships’ accidents to be basically caused by human errors. This study aims to investigate the human error of Tanker ships collision and presents the collision prevention program. Human errors were analysed by the GEMS dynamic model method adopted by IMO. The classification system, known as the “Skill, Rule, Knowledge based” approach is described in a number of publications in Appendix I, at IMO. This study analysed on the human errors of Tanker ships collision related to the negligence of look out and classified basic error type using GEMS dynamic model. The study results follow the classified basic error types; Mistake – Slip – Lapses - Violation. This, also, suggests improvement of the 6-mile rule(Masthead Light, over 50m length) required by Rule 22 of the International Regulations for Preventing Collisions at Sea(1972) due to technical development, Modernization, and Acceleration of new ship.
