노인요양병원 운영 실태는 어떠한가?

How does the long-term care hospital work? -content analysis to conceptualize patient's family and personnel's experience-

본 연구는 심층면접을 적용한 질적 연구로서 노인요양병원 환자 및 가족, 종사자들의 경험을 토대로 노인요양병원의 운영 실태를 재해석하고 풀어내는 데 목적을 두었다. 주요 분석 결과를 보면, 환자와 가족들이 노인요양병원을 이용하는 과정에서 나타나는 경험의 주제들은 습관으로 굳어진 무례함을 견딤, 치료의지가 없어짐, 간병의 사각지대에 노출됨, 기계적인 돌봄, 모욕적인 처우로 나타났다. 노인요양병원의 환경에 대한 경험적 주제는 “열악함”으로 모아졌는데 보다 구체적으로 환자에 대한 배려가 없는 점, 청결하지 못한 점, 안전하지 못한 점, 의료서비스의 미흡성으로 나타났다. 본 연구 결과로부터 파생되는 시사점은 다음과 같다. 첫째, 노인요양병원 종사자에 대한 노인환자 인권교육이 강화되어야 한다. 두 번째 치료와 돌봄을 둘러싼 노인요양병원의 기능이 재정립되어야 한다. 마지막으로 요양병원이 환경을 개선하기 위한 의료법 등 관련 기준이 개선되어야 할 것이다.

The purpose of this study was to obtain specific understanding about how Long-Term Care Hospital is managed in practical terms. With this purpose, the researchers took main research question as follows : “what do they experience and how do they feel it?” Snow ball sampling was used to contact 11 participants for the completion of qualitative semi-structured face-to-face interviews. The results of data analysis are as follows. There are two categories : 'patients and families in a predicament', 'inferior circumstances of long-term care hospital'. With these findings, the researchers summarized implication as three. There is urgent need for education on human rights of patients in the long-term care hospital. The major function of long-term care hospital, which is based on care and cure, should be clearly founded on their services. Finally, government have to make overall amendment to the laws which are related to long-term care hospital.
