양측 안와에 발생한 원발성 호지킨 림프종 1예

A Case of Primary Hodgkin Lymphoma in the Orbit

목적: 호지킨 림프종이 안와 및 안부속기를 침범하는 경우는 매우 드물고 대개 질환의 말기에 전이성 종양의 형태로 나타난다. 안와에 원발성으로 발생한 호지킨 림프종은 아직 국내에 보고된 적이 없으며, 이에 저자들은 양측 안와에 발생한 원발성 호지킨 림프종 1예를 경험하였기에 이를 보고하고자 한다. 증례요약: 64세 남자 환자가 2년에 걸쳐 크기가 증가하는 좌하안검 종괴를 주소로 내원하였다. 환자는 당뇨병 이외에 특이 기저질환이나 종양에 대한 과거력 및 가족력은 없었다. 신체검사상 좌하안검에 부드럽게 촉지되는 고정된 종괴가 있었고, 외안근 운동검사에 서 양안에서 상방 주시시 경도의 안구운동 장애가 관찰되었다. 안와 컴퓨터 단층촬영 및 자기공명영상검사에서 양측하직근과 좌하안검을 침범하는 연조직 종괴가 관찰되어 절개생검을 시행하였다. 병리 조직 검사 결과 결절 경화성 호지킨 림프종으로 진단되었다. 환자는 종양내과로 전원되어 종양의 병기결정검사를 시행하였고, 양전자방출단층촬영술에서 안와 외 침범은 관찰되지 않았다. 전신화학요법 및 방사선 치료 후 치료 14개월째인 현재까지 안과적, 전신적 합병증 및 재발 없이 경과관찰 중이다. 결론: 안와 및 안부속기에 발생한 호지킨 림프종은 드문 형태이나, 양측성으로 발생 가능하며, 전신상태가 양호한 환자의 안와에서 발생한 원발성 결절 경화성 호지킨 림프종이 전신화학요법 치료 및 방사선 치료로 관해된 증례를 보고하는 바이다. <대한안과학회지 2016;57(7):1150-1153>

Purpose: Hodgkin lymphoma involving the orbit and ocular adnexal tissue is very rare and usually appears in the form of a metastatic tumor at the end stage of the disease. Primary Hodgkin lymphoma in the orbit has not been previously reported, and herein we report a case of primary Hodgkin lymphoma occurring in the bilateral orbit. Case summary: A 64-year-old male presented with a left lower eyelid mass that increased in size over 2 years. The patient had no specific past medical history or family history except diabetes. During the physical examination, a fixed mass was gently palpated in the left lower eyelid. Mild upgaze limitation was observed during extraocular muscle movement examination in both eyes. Orbital computed tomography and magnetic resonance imaging showed soft tissue masses involving the bilateral inferior rectus muscle and left lower eyelid. The patient was diagnosed with nodular sclerosing Hodgkin lymphoma after pathological examination following incisional biopsy. The patient was transferred to the oncology department for tumor staging. Positron emission tomography showed no involvement of other organs except both orbits. After systemic chemotherapy and radiation therapy, the patient was under observation for 14 months without ophthalmic and systemic complications or recurrence. Conclusions: The authors present a case of primary nodular sclerosing Hodgkin lymphoma that occurred in the orbit. After systemic chemotherapy and radiation therapy, the patient was in good general condition. J Korean Ophthalmol Soc 2016;57(7):1150-1153
