Midface are rare in children. The soft, resilient bony structure of the jaws in children is able to sustain a considerable impact without fracture. The method of reduction and fixation should take into accout developing tooth bud and every effort should be made to damage to them The treatment of the facial skeleton is difficult because teeth are too few for the fixation. Following fracture of the infantile facial skeleton, growth defects may appear caused by damage to the growth centers, sear formation and impaired function. In this case, we perfomed the ORIF and primary closure on Le-Fort Ⅱcombined with facial deep laceration in 8 year old boy duffered from traffic accident. After 1 year we removed piate on maxilla. Then we revised the scar tissues with elliptical excision, Z-plasty, W-plasty and got the favorable result. Finally, we got good cosmetic result using scar revision without significant functional restricion, growth impairment in this case.
