For the purpose of maximal reduction of postoperative positional change, we have recently tried a technique, intentional ostectomy of posterior part of distal segment, in the bilateral sagittal split rami osteotomy(BSSRO). This study done based on 42 cases of technique of BSSRO with rigid fixation was performed in 23. cases as a control group and our technique of additional distal cutting in the other 19 cases as a test group. Horizontal and vertical change were measured to determine the amount of long-term postoperative relapse indices. The results were as follows ; 1. Relapse index of test group were lower than control group significantly in 2, 6, 12 months postopratively(p<0.05). 2. Average value of horizontal relapse index of test group was lower than control group in all periods postoperatively, but insignificant statistically. 3. Vertical relapse index of test group were lower than control group significantly in 6 and 12 months postoperatively(p<0.05). 4. Facial length relapse index of test group were significantly lower than control group in 12 months postoperatively. In conclusion, this method can be considered useful method to gain the maximum post-operative positional stability. In addition, it could be especially well adapted for the correction of mandibular prognathism accompaning open bite.
