A complete mandibular subperiosteal implant is a form of oral implant that is applicable for severely atrophied mandible. The mandibular subperiosteal implant is indicated for completely edentulous patients who can not or desire not to wear conventional complete dentures and do not have sufficient bone for endosseous implants. This article is a case report of mandibular complete subperiosteal implant which was performed using a direct bone impression technique and a design of continuous mesostructure bar. Hydroxylapatite particles were used for filling the gap between bone and the implant which was made up of surgical vitallium. Postoperatively there were minor complicatons of posterior wound dehiscence and mental nerve paresthesia. Further study will be needed in CT-scan modeling to eliminate the first stage operation for direct bone impression.
