In a strict sense, any inflammatory lesion that involves the marrow can be referred to as osteomyelitis. Osteomyelitis can occur at any age and is most common in males. Although in infancy the maxilla is usually involved, during childhood and later life the mandible is the preferred site. Aute axacerbation of periapical lesions and fractures are the precipating factors. Staphylococci and streptococci are the usual causative organisms, but osteomyelitis may be caused by other organisms such as Actinomyces israelii and tubercle bacillus. The treatment of teeth in the teeth-bearing mandibular fracture is controversial. The teeth in the mandibular fracture line can be preserved with the proper antibiotic therapy, but in the case of complication, especially impacted third molar, the teeth in the mandibular fracture line must be extracted for preventing osteomyelitis. The following report concerns rarely patient who suffered pus-producing osteomyelitis only in the left mandibular angle following both mandibular angle fractures with full impacted third molar.
