A Clinical stuay of dysesthesia following surgical extractions of the mandibular third molars: a prospective study

This is a prospecitive clinical sytudy to evaluate and predict the clinical incidence, predictable contributing factors and prognosis of dysesthesia following surgical removal of mandibular third molars. The author stuies 368 patients(414 lower third molars) who had been treated by Dept. of Oral and Maxillofacial Surgery at Yonsei University and Dept. of Dentistry at Wonju Christina Hospital during Oct, 1988 through Sep., 1989. the obtained results were as follows ; 1. The incidence of dysesthesia after surgical removal of lower third molars was 4.1% (17 of 414 teeth). 2. The predictable etiologic factors for Dysesthesia were 1) Distoangulation, 2) Radiolucent band or displacemennt of canal on the radiographs, 3) Exposed inferior alveolar canal, remarkable hemorrhage and/or unplanned root fractures during operation. 3. The incidence of dysesthesia in extracted teeth which were not related directly and/or indirectly to surgical trauma on imferior alveolar nerve, vessels ound/or canal walls during operations was 3.5% of total group and 76.5% of dysesthesis group. 4. Occurrences of dysesthesia were not correlated with sex, age, depth of teeth impatction, preoperative inflammation, postoperative swelling, and difficulty of operation. 5. The psychological states of dysesthetic patients were not different from those of normal individualls. 6. Dysesthesia after removal of mandibular 3rd molars could be resolved during 4-6 months in this study, though statistically insignificant.
