In sagittal split osteotomy as well as in vertical osteotomy for reduction of mandible, wire and bone screw osteosynthesis has been used. Relapse after sagittal split ramus osteotomy was well documented in many studies. Most of these studies, however, associated with cases which were fixed by transosseous wiring technique. For that reason, relapse rates after sagittal split osteotomy were reported to be reasonably high. Author studied of 20 patients who treated with bilateralㆍsagittal split ramus osteotomies. For reuction of mandible, stable osteosynthesis with tandem-screw was accompanied with IMF for 3 to 5 days. Author took the serial radiograph before and immediate post operative, postoperative 6 months. Author analysis 5 angular measurement and 4 linear measurement that was used to find out the amount of change in condyle position. According to the final analysis, minimal change was found in condylar position using thandem-screw fixation at postoperative 6 months. Resultly autuor s study indicates resistance against relapse with using tandem-screw fixation after sagittal split ramus osteotomy.
