치아 외상성 손상의 발생 빈도와 예후에 대한 임상적 고찰

A clinical study of the incidence and prognosis of the traumatic tooth injury

The incidence of the traumatic teeth in juries were studied retrospectively with a total of 489 patients(Total No. of teeth : 1565) and the affecting factors to prognosis were studied with 319 teeth at the department of oral and maxillofacial surgery, colloege of dentistry, Wonkwang university. According to the age and sex, etiologic factors, type of tooth injureies and the incidence were studied and the result were as follows : Male patients of second and third decades were most frequently involved and the average number of teeth injuried were 3.1. The most common type of tooth injuries were subluxation (includes concussion) among the periodontal tissue injuries and complicated crown fractures among the hard tissue injuries. The traffic accident were the most common etiologic factors. Traumatized teeth can disclose a number of complications, such as pulp necrosis, pulp canal obliteration, root resorption and/or loss of marginal supporting bone. So we investigated indirectly the prognosis with pulp necrosis, teeth mobility change, and root resorption. The incidence of pulp necrosis and its relationship with age, root formation, the degree of reduction, the time elapsed between injury and treatment were studied. Tooth mobility changes according to alveolar bone level and root resorption according to the time interval between injury and treatment were also studied. The results were as follows ; The pulp necrosis were occured in the 158 teeth (49.5%) among 319 teeth and root resorption were 29 teeth(83%) among the total avulsed 35 teeth. Root canal treatment were performed in the 172 (54.1%) teeth and 25 teeth were extracted due to severe mobility, periodontal abscess, and periapical lesion.
