Malignant giant cell tumor apparently arises from benign giant cell tumor which is a neoplasm that develops from mesenchymal cells with differentiation toward fibroblastlike stroma, multinucleated giant cells, and a misture of other cells, predominantly inflammatory. It is a low-grade malignant neoplasm which is characterized by slow restless growth, a high recurrence rate following less than adequate attempts at local removal, and a low metastatic rate (20 % or less). The giant cell tumor is characterized by a high recurrence rate following curettage or irradiation. The best success has been achieved by either en-bloc resection or cryosurgical techniques. Approximately 70 percent of patients are between 20 and 40 years old and malignant giant cell tumors occur at a slightly older age than the usual giant cell tumor. But in this case, much to our supprise, it occurs in growing child and behaves aggressively. Now we present this rare case with a review of literature.
