Internal derangement of the TMJ is defined as an abnormal relationship of the articular disc to the mandibular condyle, fossa, and articular eminence. Internal derangement of the TMJ must be differentiated from extra-articular conditions referring symptoms to the TMJ. Diagnosis of internal derangements is made by history taking, physical examination, and plain radiographic studies, but in special circumstances TMJ Arthrography, C.T., and MRI are usful. Surgery today may include meniscectomy, emniscectomy with interpositional implants, hard tissue procedure (Bone reduction), and disk repair procedures (meniscoplasty, meniscorhaphy). In advanced process of internal derangements of TMJ, surgery without prior monsurgical treatment is recommended. We surgically treated 3 patients of internal derangements of TMJ as below. Case 1; Meniscectomy under the impression of degenerative arthritis, Lt. joint. Case 2; Meniscorhaphy/high condylectomy/eminectomy under the impression of anterior disc displacement without reduction. Case 3; Meniscoplasty/high condylectomy under the impression of anterior disc displacement without reduction.
