This is to report about new-approaches for treatment of the mid-facial bone fractures. The authors treated successfully two cases of comminuted fracture of the mid-facial bones and one case of coronal and facial deformity due to craniectomy and malunited zygoma complex by coronal and temporal flap approaches. Two cases were treated by coronal flap and one case was treated by temporal flap. These approaches gave many excellent benefits to the operator and patients than other conventional methods of treatment of the mid-facial bone fractures. The results are as follows: 1. Coronal and Temporal Flap approaches were excellent for the fine reduction of comminuted fracture fragments of Zygomatico-maxillary complex and the late reduction of malunited Zygoma complex, because these offered wide operating-field to supra-and lateral orbit rims, zygomatic body and zygomatic arch. 2. These approaches gave excellent cosmetic benefits to the patients due to complete hiding of post-op. scar in hair, and were less danger to important anatomic structures as facial nerves and vessels in spite of radical exposure. 3. Pre-op. orientation and education about safety results may be necessary to the patients and relatives for minimizing of psychological phobia or anxiety to radical scalp incision. 4. These approaches will provide many benefits to the Cranio-Maxillary Orthognatic Surgery sooner or later.
