This experimental investigation was undertaken to observe the pulpal and periapical changes after the intentional vital tooth apicoectomy. 4. adult mokeys were used, and roots of maxillary & mandibular quadrants were transected within the apical third with a fissure-bur. The dentoalveolar segments for 1, 2, 4, 6 weeks following surgery were obtained and examined by routine histologic preparation with H-E stain & Preece s trichrome stain. The results were as follows; 1. At 1 week after vital tooth apicoectomy, necrosis with disruption of normal pulpal tissue and infiltration of some inflammatory cells were found. 2. At 2 weeks the transection wounds were filled with new collagen and granulation tissues. The necrotic pulp was replaced with periodontal ligament like tissues. 3. At 4 weeks most of the pulps were replaced with periodontal connective tissue and a lining of cellular cementum covering the root canal walls. 4. At 6 weeks the pulps had intact vascular supply and periodontal ligament appearance. The gradual deposition of cementum along the root canal walls and collagen fiber bundles between the new bone & the newly deposited cementum could be seen.
