상악골 전치부 분절골절술에 의한 상악골기형의 치험예

A case report of surgical correction of maxillary deformity by the anterior maxillary segmental osteotomy

A male patient of anterior maxillary protusion at the age of 24, who complained of phonetic problem, esthetic problem and edge to edge bite due to open bite and had congenital missing of both mandibular canine, visited the Department of Oral Surgery, Army 00 Evacuation Hospital, and was treated by anterior maxillary segmental osteotomy involving midpalatal splitting and extraction of both maxillary first premolar. The results are as follows; 1. Selection of the treatment is based on a clinical study of facial status of the patient, and analysis of study model and cephalometric radiographs. 2. A correct diagnosis and proper treatment improve mastication, phonetic and esthetic complaints. 3. The postoperative analysis don t reveal changes of pulp and root involving teeth of osteomized segments. 4. Common sequelas such as swelling and ecchymosis disappeared in 3 to 10 days after surgery, and the paresthesia of upper lip was recovered in 3 to 6 months later. After 6 months of treatment, postoperative radiographs and clinical examinations showed consolidation of osteomized segments without mobility. There has been no discernible change in the position of operation sites during 6 months of postoperative follow-up.
