This is a case of orthognathic surgery for mandibular protrusion. In the surgical correction of mandibular protrusion, many methods have been used in diverse cases. But, for the correction of severe protrusion of the mandible, the combination of two or three surgical methods is desirable and inevitable. Recently this tendency of combination of surgical techniques is growing in the field of orthognathic surgery. In this case, mandibular step osteotomy and sagittal split osteotomy were used for correction of the discrepancy of 15mm in the mandible. This combination in the correction of protruded lower jaw is indicated when followings exist simultaneously. * More than 10mm in length should be reduced un the mandible and horizontal ramus is very long. * The size of lower dental arch is larger than That of the upper dental arch and there is a tendency of posterior crossbite. * It is necessary to elevate the mandibular body in the anterior portion because of anterior open-bite. But skillful and exquisite technique of operation is inevitable to avoid the complications from these two techniques.
