口腔內 扁平上皮細胞癌에 關한 臨床的 硏究

Clinical Study of Intraoral Squamous Cell Carcinoma

Between January 1979 and September 1982, 123 patients with squamous cell carcinoma in oral cavity were studied clinicopathologically, and following results were obtained. 1. The ratio of incidence between male and female patients was 2.3:1. Most of the cases occurred in the age of 40 to 69. (77.3%) 2. In the degree of histopathologically differenciation of intraoral squamous cell carcinoma, well-differenfation comprised 60.2% of 123 cases. And 56 patients hospitalized for the treatment of intraoral squamous cell carcinoma were studied clinically as for factors affecting diagnosis and prognosis, and following results were obtained. 3. 64.3% of 56 patients and 85% of male patients used cigarette. 4. Most common symtom of patients at the admission was pain, followed by swelling, ulceration, and neurologic disturbance. 5. Time elapse from the recognition of symtoms to final diagnosis were 8.6 months in average, and during this period, involved teeth were extacted in 24 patients (42.8%) of 56 patients. 6. The sequence of prevalent site was lower alveolar mucosa (33.9%), upper alveolar mucosa(21.4%), tongue, buccal mucosa, floor of the mouth, palate. 7. In size of intraoral squamous cell carcinoma, tumor size between 2cm and 4cm comprised 62.5% of 56 cases. 8. Cervical lymph node metastasis was clinically suspected in 55.4% of 56 cases, and most commonly involved lymph nodes were submandibular and subdigastric lymph nodes. Distant metastasis was not suspected clinically or radio-graphically in all cases at the admission. 9. According to TNM classification, Stage III and Stage II comprised 42.8% and 30.4% respectively. 10. In the modalities of treatment, block excision and neck dissection were done in 67.9% of 56 cases, and 47.4% of them were recieved postoperative irradiation therapy.
