An unusual case of ossifying fibroma is presented. The patient had clinical and radiologic fibrous dysplasia in the left mandible, but a biopsy showed abundant trabeculae rimmed by plump osteoblasts which was a classic microscopic feature of ossifying fibrona. The lesion was removed by partial mandibulectomy and the defect was replaced with a temporary acrylic resin graft. There was persistent pus discharge from the connection area between the bone and the graft for 7 months, and it urged us to remove the graft. Corrosion on the surface of the metal plate and the screws, which was found on the second operation, might have acted as one of the factors of delayed healing. Follow-up radiographs disclosed bone regeneration along the inner aspect of the graft and it is believed that chronic infection probably played a role in activating the new bone formation.
